About this file

This is Addendum No. 3 to RFP 25 8302 DPS issued by the Orlando Utilities Commission (OUC), a Florida-based utility provider. The addendum addresses questions received during the solicitation process and revises the original RFP for post-hire occupational health assessment services. OUC is seeking qualified suppliers to provide comprehensive occupational health evaluations for employees across six locations in the Orlando metropolitan area, St. Cloud, and Titusville, Florida. Services include respirator medical questionnaires, asbestos assessments, quantitative fit tests, electrical resistivity tomography evaluations, and hearing audiometric tests. Proposers may offer either mobile onsite medical units, a brick-and-mortar facility within the Orlando metropolitan area, or both service delivery models. The original proposal due date of July 16, 2025, at 2:00 PM EST has been extended to July 30, 2025, at 2:30 PM EST. The contract term is three years with two potential one-year renewal options. Proposers must acknowledge receipt of this addendum in their submittal or risk being considered non-responsive.

The addendum does not contain pricing or financial information; these details remain in the original RFP. No M/WBE set-asides are required. All prospective proposers must submit required forms including the Exceptions form, Sworn Statement, Dispute Disclosure, Form W-9, Technical Risk Questionnaire, Standard Contract, OUC Contractor Security Policy, Bid Protest Procedure, OUC Generic Contractor orientation materials, and Drug and Alcohol-Free Workplace and Testing Policy. Proposers with questions should contact the buyer of record, Dom P. Sotero, CPPO, CPPB, Senior Buyer, at dsotero@ouc.com. The addendum emphasizes that no communication regarding this solicitation may occur between proposers, their employees, or subcontractors and any OUC personnel other than the designated buyer of record.
