About this file

This is Addendum 6 to the Request for Proposals (RFP 25598-DST) issued by Hillsborough County Public Schools (HCPS) in Tampa, Florida, for medical benefits and prescription drug services. The addendum comprises questions and answers clarifying RFP requirements for comprehensive medical coverage, including behavioral health and pharmacy services, for approximately 19,121 enrolled participants across active employees, under-age-65 retirees, Medicare-eligible retirees, and COBRA participants. HCPS is soliciting proposals under four primary options: self-funded Administrative Services Only (ASO) with pharmacy carved in or out, and self-funded Third-Party Administrator (TPA) with pharmacy carved in or out, with an additional option for pharmacy-only (carved-out) proposals. The deadline for technical questions is November 20, 2025, with proposals due December 12, 2025, through VendorLink. The Insurance Committee is scheduled to review RFP responses on January 27, 2026, with board approval anticipated in February 2026 and contract execution estimated prior to January 1, 2027. Implementation will commence in March 2026, open enrollment is scheduled for September 2026, and the contract effective date is January 1, 2027.

The addendum addresses critical requirements for vendor qualification, including pre-qualification submission deadlines extended to November 13, 2025, at 5 PM ET, with minimum qualifications for pharmacy benefit managers including five years of experience in Florida and registration with the Florida Office of Insurance Regulation. HCPS will transition from a fully insured Aetna model through December 31, 2025, to self-funding effective January 1, 2026, with three new medical plan designs. The wellness program budget totals $2.85 million, which respondents must fund directly on HCPS's behalf, including the Vitality4u program, incentives, biometric screenings, and other associated costs. The RFP evaluation will use a 100-point scoring system with 45 points for cost/value, 15 points for plan administration, 20 points for service capabilities, 15 points for network access, and 5 points for related experience. Federal funding sources support this procurement, requiring compliance with Appendix II to 2 CFR Part 200, and the Student Nutrition Services program is funded by USDA, with specific telecommunications and foreign country affidavit requirements applicable. HCPS will not pay cost recovery for custom implementation or capital investments in the event of early termination for convenience or funding non-appropriation.

Addendum #: 6

NOTICE TO ALL POTENTIAL BIDDERS/PROPOSERS/RESPONDENTS

The above-referenced competitive solicitation is modified as outlined in this addendum. The original competitive solicitation documents and any previously issued addenda remain in full force and effect, except as modified by this addendum, which is hereby made part of the competitive solicitation. Respondents must consider this Addendum when preparing and submitting their bid/proposal/quote/reply.

1. Questions and Answers

The following questions and answers are provided as a matter of information to clarify issues raised regarding the competitive solicitation. To the extent that changes to the competitive solicitation are required based on the questions received, the competitive solicitation has been modified as noted above in the competitive solicitation section of this addendum.

Question Answer Per the RX RFP Announcement it states that Vendors who would like to submit for Options 1 and Options 3 must be prequalified and submit Attachment A and Attachment C by November 11, 2025.

Question: On the Attachment A can you confirm if Vendors just need to Complete the Proposal Confirmation tab by the November 11, 2025 due date or are you also wanting Vendors to provide the Required Attachments listed on Tab 4. Additional Information. If Vendors are to also complete the Tab 4 can you please provide the Geo-Access Parameters (2 in 5, 2 in 10, etc) that you would like to see reports ran on as that is not included in any of the files attached in Vendorlink.

Proposers need to complete Tab 5, Proposal Confirmation. Tab 4 is information, and required attachments can be submitted with the final proposal.

For the Geo-Access, please use 2 in 10.

Please advise based on the census file Medical plan codes which plans they align to for the 2026 plan designs so we can ensure we are capturing the correct membership in each plan Codes from Census HDHP, Local, National, Select, Staff Plan Designs: High PPO, Low PPO, HMO

Please reference the revised census. See Addendum 2.

Use the password noted in Attachment A – Medical-Rx Read Me First.

Please provide a revised Cobra census that also provides the members Gender and Zip Code

If we are bidding with the Rx carve out options, do we need to submit the proposal confirmation tab and/or Attachment C, by 11/11?

Attachment C must be completed if the respondent is submitting a proposal that includes pharmacy services, which could be provided under a carved-in or carved-out scenario. In a carved-in scenario, one carrier would provide both medical and pharmacy. In a carved-out scenario, a respondent could quote on medical services and partner with a pharmacy benefit manager (PBM) to offer the pharmacy services. In other words, the only time a respondent would not need to complete Attachment C is If they are only quoting medical services. Refer to Addendum 3.

Our PBM partner has 2 public sector clients over 20,000, one of which is over 100,000 covered lives. Would this be disqualifying for the Rx carved in bid? Given that the form is due tomorrow, if the PBM partner is not accepted, would we still be able to respond as TPA only with Rx Carve out?

If a respondent has a separate PBM partner, they will need to quote under one of the carved-out options. HCPS is looking for a complete solution that includes services for medical, behavioral health, and pharmacy – there is flexibility on whether these services are provided by one company or provided by two companies that are partnering together. If a vendor/PBM does not meet the minimum qualifications as stated in Attachment C, that vendor/PBM will not be accepted.

The deadline to submit the documents to pre-qualify for the Rx Only RFP has been extended and is set to 11/13/2025 at 5 pm ET. Refer to Addendum 3.

How does HCPS define carved in vs carved out? Carved-in – One company provides both medical and pharmacy services.

Carved-out – One company provides only medical services, and a separate Pharmacy Benefit Manager (PBM) provides only pharmacy services.

When will HCPS be making a decision on a carrier as this information would be needed to complete the Implementation Timeline

The deadline for the Rx Minimum Requirements is 11/13/25 at 5 pm. HCPS will respond to each individual who submitted no later than the week of 11/17/25.

The Insurance Committee is scheduled to meet on 1/27/26 to review all RFP responses that met minimum requirements and evaluate accordingly. The voting results will determine which vendor/solution is recommended to award.

A draft implementation timeline is acceptable.

Can you confirm the revised due date of the RX Only RFP deadline as the Addendum 3 states 11/15 but the response in questions submitted by Vendors states 11/13

The deadline to submit documents to prequalify for Rx Only RFP is 11/13/2025, 5 PM. Please refer to Addendum 4.

Our PBM partner only has 2 public entity clients over 20,000 lives. However, one of these has over 100,000 members. Would this level of experience be acceptable, or would this be disqualifying for the PBM?

If a vendor/PBM does not meet the minimum qualifications as stated in Attachment C, that vendor/PBM will not be accepted.

Would HCPS be willing to work with a partner who proposes only medical services, and has the ability to integrate with various PBMs?

HCPS is accepting bids for both carved-in and carved-out solutions. Both medical carriers and PBMs can respond individually.

Would HCPS be willing to work with a partner who proposes only medical services, and has the ability to integrate with various PBMs?

Duplicate question - HCPS is accepting bids for both carved-in and carved-out solutions. Both medical carriers and PBMs can respond individually.

Hillsborough County Public Schools Page 3 of 15

We plan on bidding self-funded TPA services only, Rx carved out. Just to be clear do we need/are we expected to respond to the SB1550 question on the proposal confirmation tab, given that we are not proposing PBM services?

Yes. HCPS would like to know how your interpretation of SB1550 related to medical pharmacy.

Please advise if RX data files will be available for the following items: NDC-11 Fill Date Pharmacy ID (NABP/NPI) Retail/Mail Indicator Days Supply Quantity

Hillsborough County Public Schools Page 9 of 15

Below are the questions we have compiled:

1 "Section 1.4 lists multiple options (ASO vs. TPA; Rx carved in vs. carved out).

Does HCPS intend to make one combined award for Medical + Rx, or is HCPS open to separate awards (e.g., medical to one vendor and pharmacy to another)? If separate awards are possible, will HCPS coordinate a joint implementation between awarded vendors?"

2 Is HCPS anticipating a single primary vendor for all medical/Rx services, or is there any scenario where multiple vendors could be awarded (e.g., by population segment or plan type)?

3 "Please provide the anticipated Board approval date, contract execution date, and target dates for:

Implementation kickoff Open enrollment communications Final system readiness / testing prior to the January 1, 2027 go-live."

4 Section 2.5.2 references optional Phase 2 presentations. Can you confirm whether HCPS expects to hold finalist presentations, and if so, during what anticipated week(s)?

1. Respondents have the opportunity to respond to just medical, just pharmacy, or both. A joint implementation would be expected if there are two companies chosen.

2. This will be up to the committee, but HCPS has not discussed or considered separating vendors by population or segment at this time. See Sec. 2.6 of the RFP.

3. Board approval date is tentative for February 2026.

Contract execution date is estimated to be prior to 1/1/2027. Implementation will kick off in March 2026.

Open Enrollment is scheduled for September 2026 with communications kicking off in June.

HCPS will partner with the chosen carrier to work backwards on system readiness and testing prior to 1/1/2027.

4. BAFO proposals are estimated due on 1/29/26 with vendor presentations scheduled, tentative date of 2/3/26. Finalist negotiations are estimated to conclude on 2/6/26.

5 Section 1.5 lists active employees, under-65 retirees, Medicare-eligible retirees, and COBRA participants with total enrollment as of August 2025. Please confirm that these are all populations in scope for this RFP (including any charter schools, leased employees, or other affiliates). Can HCPS provide projected enrollment by year for at least 2027–2029, or planning assumptions we should use?

6 Please provide or confirm current eligibility rules (full-time threshold, waiting period, re-hire rules, seasonal/temporary staff treatment, etc.) and whether any changes are anticipated for 2027.

7 Are Medicare-eligible retirees currently enrolled in a Medicare Advantage / group supplemental plan, or in the same commercial plan(s) as actives? Should proposers assume an integrated solution for Medicare-eligible retirees, or is HCPS open to a separate Medicare plan arrangement?

8 "Please provide additional detail on the claims experience period(s) provided (start/end dates, any large-claim pooling, and whether data is incurred/paid/lagged).

Will HCPS provide 2026 self-funded experience under Aetna as soon as credible, and will any such update be shared with all proposers?"

9 The narrative in Section 1.5 provides membership counts as of August 2025. Please confirm that the membership totals in the census align with these counts, and if not, provide a reconciliation or guidance on which source should be treated as authoritative for rating.

5. Confirmed that these are all populations in scope for this RFP (including any charter schools, leased employees, or other affiliates). Additionally, Leave of Absence employees should be included. Enrollment projections are unavailable.

6. See tab 1. Read Me First in Attachment A for eligibility rules. No anticipated changes for 2027.

7. Currently, post-65 retirees have the option to choose to stay on HCPS's active plan, paying the full premium, or choose a sponsored Medicare plan.

Please refer to the census for medically enrolled retirees.

This RFP does not include Medicare Advantage Retirees.

8. The data provided included paid claims from 2023 (Humana), 2024 (Aetna), and data through September 2025 (Aetna). The decision of this RFP will be final prior to 2026 self-funding experience being made available.

9. Please use the census counts. The numbers in section

1.5 don't include total members.

Hillsborough County Public Schools Page 10 of 15

10 Will HCPS provide standard provider and Rx disruption files (by NPI, Tax ID, or NCPDP for pharmacies) to enable accurate disruption analysis aligned to the evaluation criteria in “Network Access and Disruption”?

11 Does HCPS expect the awarded vendor to arrange and price specific/aggregate stop-loss, or will stop-loss be procured separately? If vendor-arranged stop-loss is desired, please provide any specific attachment points, aggregate factors, or corridor preferences.

12 "Section 2.5.1 lists “Rate Guarantee/Cap” and “Performance Guarantees” under the Cost/Value criteria.

Does HCPS have a preferred rate guarantee period (e.g., 3 years) and maximum acceptable annual trend caps for admin fees and Rx pricing?

Should proposers price admin fees separately for medical and pharmacy, or as a combined PEPM?"

13 Please clarify which types of credits HCPS would like proposers to quantify separately (e.g., wellbeing, communication, implementation, ongoing audits) and whether HCPS has any minimum expectations for these items.

14 For carve-in and carve-out scenarios, is HCPS seeking transparent pass-through pricing (with separately stated admin fees and rebates), or will HCPS also consider spread-based models?

Self-Funded ASO option with Rx carved in; 2. Self-Funded ASO option with Rx carved out; 3. Self-Funded TPA option with Rx carved in; 4. Self-Funded TPA option with Rx carved out. This is listed in another section as well.

However, in the RFP Instructions tab, there are five options listed: Self-Funded ASO, Rx Carved In; Self- Funded TPA, Rx Carved In; Self-Funded ASO, Rx Carved Out; Self-Funded TPA, Rx Carved Out, Rx Only (Rx Carved Out).
