About this file

This is Addendum #5 to a Request for Qualifications (RFQ #C15E0055) issued by the California Department of Parks and Recreation for Site Characterization and Remediation Services for Hazardous Materials at Malakoff Diggins State Historic Park. The addendum extends the Statement of Qualifications (SOQ) submittal deadline to 2:00 p.m. on May 9, 2016. The project involves site characterization and remediation services for hazardous materials throughout the park site.

The addendum clarifies that approved work orders will include task descriptions and agreed-upon lump sum costs per task, with contractors required to submit proposals including estimated labor hours and rates prior to work order issuance. The Schedule of Rates requested in Section 3.1.2 D is noted as a minimum requirement but will not be used in the selection criteria evaluation. The document confirms that example projects listed in SF 330 Section F can be used to demonstrate successful project completion and value matrix requirements. Additionally, the addendum specifies that public work contracts funded with state funds require prevailing wages for construction services. A mandatory pre-proposal walkthrough attendance list is available on Cal eProcure.

1. CLARIFICATION: SOQ submittal deadline is extended to 2:00 p.m., Monday, May 9, 2016.

2. Questions (Q) and Answers (A) submitted:

1) Q: Based on the information provided in the RFP, it is unclear if the contract structure for individual work orders will be lump sum or time and materials (T&M). Section 1.1.3 of the RFQ describes that “Contractor’s estimate shall include…all estimated hours of labor and the corresponding hourly rate…” but Section 1.2.1 [C] states that, “The method of compensation shall be on a fixed fee…”. Can DPR clarify the intended contract structure (i.e. lump sum, T&M, T&M not-to-exceed, cost plus, etc)?

A: Prior to issuing a work order Contractor shall submit a proposal for the scope of work which will include an estimate including all estimated hours of labor and corresponding hourly rate. Approved work orders will include task(s) description(s) and agreed upon lump sum cost per task(s).

2) Q: Rates and fees are not listed in the selection criteria in Section 3.2 of the RFQ. Can

DPR clarify how the schedule of rates requested in Section 3.1.2 [D] will be used/evaluated?

A: The RFQ is not a Fee Proposal. The “Schedule of Rates” is a “Minimum Requirement per Section 3.1 Minimum Requirements, paragraph 3.1.2 D. The “Schedule of Rates” is not a selection criterion and will not be used or evaluated in the selection process.

3) Q: Section 3.1.2 [C] requests that, “Contractor shall include examples of successful projects completed within budget.” Will DPR consider the example projects listed in SF 330 Section responsive to this request?

Addendum #5 RFQ Bid Number: C15E0055

A: Yes

4) Q: Section 3.1.2 [E] requests a value matrix for equivalent type work performed. Will DPR consider the example projects listed in SF 330 Section F responsive to this request?

A: Refer to Addendum #4
