About this file
Addendum 4 is a document from Brevard County Purchasing Services related to the Brevard County Group Dental Plan (RFP P-1-25-18), issued on May 29, 2025. The addendum provides clarifications and responses to various bidder questions about the upcoming dental insurance procurement. Key details include confirmation that the county has approximately 4,000 benefit-eligible employees and 1,000 benefit-eligible retirees, with proposals sought for fully insured group dental plans covering active employees, retirees, and eligible dependents. The proposal opening date remains Wednesday, June 4, 2025, at 4:00 pm, with the county intending to award a single dental carrier for the requested coverages.
The addendum reveals that the county is conducting a standard market check due to the current contract's expiration, seeking potential enhancements in provider access, member service experience, and cost competitiveness. Notable points include no plan design changes in the past 36 months, a 30-day grace period for premium payments, and electronic distribution of Summary Plan Descriptions. The county has specifically noted member complaints about provider access and appointment availability with the current Cigna DHMO network, indicating interest in proposals offering expanded DHMO networks or enhanced access features. The county expects the new dental plan to closely match current Cigna offerings, including exclusions and limitations, with any system deviations to be clearly identified in the proposal.

P-1-25-18 May 29, 2025 Addendum 4
TO ALL PROSPECTIVE BIDDERS
This is Addendum 4 and shall be considered as part of the original proposal package for the above-mentioned bid. Please acknowledge receipt of Addendum 4 in the space provided on the Price Sheet.
CLARIFICATION TO QUESTIONS ASKED RECEIVED:
1. Question: Can you confirm total number or employee and retirees that are benefit eligible?
Response: Based on the census data out of CBIZ, the County has approximately 4,000 benefit eligible employees and 1,000 benefit eligible retirees as of the date of this RFP.
2. Question: Does this include the waiving?
Response: That includes everyone that is benefit eligible, regardless of whether they elected coverage or not. The census information provided shows who is actually enrolled. The answer above is more general and provides the number of persons eligible.
3. Question: Just to confirm, we will not be able to obtain a census with those who are not enrolled currently? The census we received has about 4200 employees and retirees so there are 800 employees/retirees that are waiving the coverage.
Response: The county is seeking proposals for fully insured group dental plans for active employees, retirees, and eligible dependents.

4. Question: Have there been any plan design changes over the past 36 months? If so, please provide the details and dates of any change(s).
Response: no changes.
5. Question: How frequently is the incumbent’s Out-of-Network reimbursement basis updated?
If not annually, when was the last time it was updated?
Addendum 4 – Page 2
Response: Out of network is set up with Maximum Allowable Charge (MAC). Cigna does not update OON reimbursement/MAC.
6. Question: Please provide 36 months of PPO experience by plan.
Response: The experience reports provided in this RFP includes all available information.
7. Question: In the Cigna monthly claims data, do the paid dollar amounts include anything for network charges, shared savings, retained fees, or any other dollars that were not paid to the dentists or patients? If the Cigna monthly claims data include dollars for network charges, shared savings, retained fees, etc., please provide the monthly claims dollars without any such charges, savings, or fees.
Response: Shared Savings is not applicable to Cigna’s DPPO plans with MAC.
8. Question: Does Brevard County Board of County Commissioners expect to pay premiums within a 30-day grace period?
Response: A 30-day grace period from the due date is acceptable.
9. Question: Is Cigna currently mailing ID cards and Certificates of Coverage to employee homes together?
Response: ID cards are mailed. Certificates of Coverage/Summary Plan Descriptions are electronic. For new enrollment Cigna mails ID cards. Cigna does not mass re-issue ID cards each year.
10. Question: How often are SPDs mailed?
Response: SPDs are provided electronically each year; they are not mailed.
11. Question: How is Cigna handling replacement ID cards?

12. Question: How is Cigna handling replacement Certificates of Coverage?
Response: Certificates of Coverage/Summary Plan Descriptions are electronic.
13. Question: Proposer is to have NO minimum participation requirements & must permit retired
EEs & eligible dependents to continue coverage under County plans.
o If United Concordia Dental (UCD) can include a comment in our proposal that we are the only dental carrier offered for coverage, then we can agree to this stipulation.

o UCD Underwriters: Solistice—DHMO has a contracted partnership with United Concordia
Dental to work procurements & commercial request for proposals as needed. Confirm if UCD’s partnership with Solistice DHMO be considered to be a part of UCD “Prime Contractor” or considered as a subcontractor? If Solstice is identified as a 3rd party contractor to UC Dental, please advise what is required for official approval to proceed.
o Please Confirm:
Response: Confirmed.
15. Question: Cigna PPO Low Plan Cert states CHILD ONLY Ortho coverage while the Cigna
PPO Low Plan cert states ADULT & CHILD.
• Cigna PPO High Plan Cert does NOT state CHILD ONLY or ADULT & CHILD ortho, while the Cigna PPO High Plan states ADULT & CHILD ORTHO o Please Confirm status of both options for accuracy.
Response:
• Cigna Low PPO = ortho covered as Class IV Orthodontia applies only to a Dependent
Child less than 19 years of age. Subject to lifetime benefit limit.
• Cigna High PPO = ortho covered as Class IV Orthodontia for adults and children regardless of age. Subject to lifetime benefit limit.
• Cigna DHMO = ortho covered as Class IV Orthodontia for adults and children up to 19th birthday. Subject to DHMO coverage limitations.
16. Question: Any other issues at hand that may need to be addressed due to contract ending & market check for a potential carrier change?
Response: The County is conducting a standard market check as part of its due diligence process due to the expiration of the current contract. As part of this review, the County is open to evaluating potential enhancements in provider access, member service experience, and cost competitiveness. A potential carrier change would require a smooth implementation plan to minimize disruption to employees, particularly those enrolled in DHMO plans where provider continuity is a key concern.
17. Question: Any benefit features that are not currently offered that would be of interest due to noise experience by members?
Response: The County has received member complaints specifically related to provider access and appointment availability under the current Cigna DHMO network. As such, proposals that offer an expanded DHMO network or enhanced access features (e.g., more flexible provider selection or easier appointment scheduling) would be of interest.
18. Question: Group benefits are currently being matched along with matching Cigna exclusions & limitations per request and system deviations will be identified.
Addendum 4 – Page 4
Response: Correct. The County expects benefits to be matched as closely as possible to the current Cigna offerings, including exclusions and limitations. Any system deviations should be clearly identified in the proposal for County review.
19. Question: The Mandatory Features Checklist states that it and Attachment B – Proposal
Worksheet should be completed and submitted in WORD format. Can you please provide both in WORD?
Response: see attached WORD documents.
Please note that the proposal opening date and time remains Wednesday, June 4, 2025 @ 4:00 pm.
All other terms and conditions remain unchanged.
Sincerely, Mary Bowers Purchasing Manager
