About this file

This document is a Participating Addendum for the State of North Dakota's State Contract 255-1, specifically covering On-Site Vending Services and Facility Maintenance, Repair & Operation (MRO) Supplies. The addendum is a template designed for a specific purchasing agency to enter into a contract with a contractor, detailing the terms and conditions for vending and supply services within North Dakota state facilities.

The contract covers on-site vending services, with specific provisions for product inventory, delivery, billing, and reporting requirements. Key contractual elements include comprehensive insurance requirements (commercial general liability, automobile liability, workers compensation) with specific minimum coverage limits, and an indemnification clause protecting the state. The document outlines detailed insurance specifications, including policy ratings, additional insured status, and waiver of subrogation. While specific pricing is not detailed in this template, the addendum provides a framework for establishing a vending and supply service agreement that can be customized by individual state agencies.
