About this file
This is a Limited Mold Assessment Report for three buildings at the proposed VA Hospital Garland in Garland, Texas, conducted by Terracon Consultants, Inc. in June 2019. The assessment evaluated three facilities: the Main Hospital (2300 Marie Curie Boulevard), the Plaza Building (700 Walter Reed Boulevard), and the Storage Building (900 North Shiloh Road). The evaluation was limited to readily observable and accessible surfaces, with visual inspections and moisture content measurements conducted across multiple floors and areas of each building. The assessment identified extensive moisture damage from historical rain water intrusion and mechanical/plumbing leaks, along with suspect visible fungal growth in numerous locations.
The assessment documented significant fungal contamination exceeding the 25 square foot regulatory threshold established in Texas's Mold Assessors and Remediators Administrative Rules (MARAR) in specific areas, including Main Hospital Rooms 309, 312B, 1136, 1131, and the Northeast Large Mechanical Room; Plaza Building's Day Surgery area, AHU P-05, and basement storage; and various other locations. Surface samples were collected and analyzed, identifying fungal species including Stachybotrys, Chaetomium, Cladosporium, Memnoniella, and Ulocadium. The report recommends identifying and repairing moisture intrusion sources, replacing moisture-damaged materials, and conducting fungal remediation by licensed Texas Department of Licensing and Regulation (TDLR) contractors for areas exceeding the 25 square foot threshold. Additional recommendations include destructive investigation of air handler ductwork by a licensed Mold Assessment Consultant and asbestos surveying prior to remediation activities.

LIMITED MOLD ASSESSMENT REPORT
Proposed VA Hospital Garland – Three (3) Buildings
2300 Marie Curie Boulevard
1.0 PROJECT DESCRIPTION
Terracon Consultants, Inc. (Terracon) conducted a limited mold assessment at the proposed VA
Hospital Garland located at 2300 Marie Curie Boulevard, 700 Walter Reed Boulevard, and 900
North Shiloh Road in Garland, Texas. The limited mold assessment conducted on June 10 – 19, 2019 was performed by the following Terracon personnel:
◼ Mr. Andrew Cassidy, a Texas Department of Licensing and Regulation (TDLR) Mold
Assessment Consultant (License MAC1496), ◼ Mr. Matthew R. Mooney, a TDLR Mold Assessment Consultant (License MAC1362), and
◼ Mr. George Slay, a TDLR Mold Assessment Consultant (License No. MAC1107).
The mold assessment was limited to readily observable and accessible surfaces. Based on information provided by the client, the buildings are proposed for renovation.
1.1 Scope of Work
The assessment consisted of a visual evaluation of accessible areas within the subject buildings.
Additionally, a visual evaluation of accessible areas within a representative number of air handling units was conducted. Measurements of moisture content of suspect building materials were also conducted.
Terracon made reasonable efforts to assess suspect materials on the interior of each building;
however, the following limitations were encountered at the time of the survey:
◼ Locked rooms throughout the interior of each building/one set of keys with property management, CBRE, on-site. Additionally, CBRE did not have access to several key-pad doors, ◼ Expedited schedule/access limited to the schedule of building security, ◼ Vacant areas of each building used for storage of remnant furnishings, ◼ Operational mental health clinic on the fourth and fifth floors of the main building, and
◼ Multiple occupied medical clinics in the plaza building.

Responsive ■ Resourceful ■ Reliable 2
Due to the referenced limitations encountered at the time of the survey, access to various areas throughout each building was limited and the potential exists for additional suspect fungal growth or water damaged materials to be present in those inaccessible areas.
Terracon assessed multiple areas above the drywall and lay-in ceiling systems in each building, where possible, but did not observe additional suspect fungal growth except where noted in this report; however; due to occupancy, building condition, and building usage at the time of the survey, access to areas above the ceiling was limited and the potential exists for additional suspect fungal growth or water damaged materials to be present in these inaccessible areas.
Terracon assessed multiple areas of the crawlspace, where possible, but did not observe additional suspect materials except where noted in this report; however, due to the condition of the crawlspace, access was limited to the standing areas and the potential exists for additional suspect fungal growth or water damaged materials to be present in these inaccessible areas.
1.2 Standard of Care
The evaluation was conducted at the subject buildings on June 10 - 19, 2019, based on information provided to Terracon regarding building conditions. This investigation was conducted in a manner consistent with the level of care and skill ordinarily exercised by members of the profession currently practicing under similar conditions in the same locale.
The results, findings, conclusions, and recommendations expressed in this report are based on conditions observed during our June 10 - 19, 2019, assessments. Many factors such as weather conditions, building occupancy, ventilation patterns, and seasonal variations in fungal concentrations can affect the conditions observed. The information contained in this report should not be relied upon to represent conditions that existed previously or at a later date. Terracon does not warrant the services of regulatory agencies, laboratories, or other third parties supplying information that may have been used in the preparation of this report.
1.3 Reliance
The report has been prepared on behalf of and exclusively for use by Enapay, LLC for specific application to their project as discussed. No other individual or entity may rely on this report without written permission of Terracon and Enapay, LLC. Reliance on this report by Enapay, LLC and all authorized parties will be subject to the key understandings and limitations stated in the proposal, this report, and Terracon’s Agreement for Services. The limitation of liability defined in Terracon’s
Agreement for Services is the aggregate limit of Terracon’s liability to Enapay, LLC and all relying parties.

2.0 EVALUATION CRITERIA
2.1 Visual Evaluation
Indoor air quality can be degraded due to multiple contaminant sources from within a building or from the outdoor environment. If contaminant sources are not controlled, indoor air quality issues that affect occupants can arise, even with well-maintained heating, ventilating, and air conditioning (HVAC) systems. Based on the multiple sources of potential indoor/outdoor contaminants that can affect indoor air quality, the visual evaluation is conducted to assess general indoor hygiene, building maintenance practices, HVAC system design and hygiene, moisture intrusion and uncontrolled condensate formation. Indoor air frequently contains a mixture of pollutants at low concentrations that are well below regulatory occupational exposure limits. The visual assessment focuses on collecting primarily observational data (i.e., information obtained by visual inspection of the building and interviews with the building management, owners, and occupants).
Since many building materials can provide a source of nutrients for fungal growth, moisture is the limiting factor in minimizing the potential for fungal growth. Therefore, visible fungal growth, excessive dirt and water-damaged building materials should not be observed. When these conditions are observed, it indicates a need to conduct additional assessment, cleaning, and/or remediation activities.
2.2 Moisture Intrusion
Elevated fungal bioaerosol concentrations in indoor environments can occur when excessive moisture and a nutrient source are present, especially in closed or concealed areas that trap moisture and have minimal air disturbance. The cellulose in the paper surfacing of drywall or gypsum wallboard provides an adequate food source for the growth of many fungal species. On-site moisture measurements are obtained using a direct-reading instrument or infrared thermography. Results from similar materials in non-suspect locations, such as non-impacted drywall walls or ceiling surfaces, are compared and used to assess the likelihood of fungal growth conditions in suspect or historically water-damaged materials.
There are no current regulatory standards that limit moisture levels in walls or building materials, but the data can be valuable when results from different locations are compared.
METHODS
3.1 Visual Evaluation
The visual evaluation was conducted in general accordance with regulations published by the TDLR
(Mold Assessors and Remediators Administrative Rules, 2017), and consensus guidelines from the
American Conference of Governmental Industrial Hygienist (ACGIH, Bioaerosols; Assessment and
Control, 1999) and American Society of Testing and Materials (ASTM, Standard Guide for Assessment of Fungal Growth in Buildings, Method D7338-14). The indoor environment and internal
Responsive ■ Resourceful ■ Reliable 4 components/surfaces in representative air handler units (AHUs) were evaluated for visual evidence of moisture intrusion, visible fungal growth or other hygiene concerns that could impact indoor air quality.
Destructive sampling or disassembly of mechanical enclosures was not within the scope of work for the project.
Terracon collected surface samples for microscopic analysis using Environmental Monitoring Systems
(EMS) Mold Tape Slide™ samplers. The samplers were touched to the surface of interest and then placed in a plastic shipping container. Seventeen (17) samples were collected from various surfaces exhibiting suspect fungal growth. Criteria to establish fungal growth on the substrate sampled were microscopic confirmation of fungal somatic and reproductive structures. Surface samples were submitted under chain of custody protocol to Terracon-Fort Worth, a TDLR-licensed Mold Analysis
Laboratory (LAB1010).
3.2 Moisture Detection
A FLIR, Model C-2, thermal imager was used to evaluate temperature differences in building materials that would be consistent with moisture intrusion.
Moisture content of building materials was evaluated with a digital moisture meter; Protimeter Mini
BLD2000, from Amphenol Thermometrics, Inc. The moisture meter was calibrated prior to use in the field. Normal moisture content of building materials was standardized by evaluating similar building materials that were not subjected to atypical moisture conditions.

Interior Ceilings
Textured drywall and 2’x4’ and 2’x2’ lay-in ceiling tile suspended from a concrete deck; spray-on fireproofing was observed in various areas throughout the main hospital on the concrete deck and on the metal decks of the stairwells
Interior Floors Unfinished concrete, terrazzo, floor coating, resilient floor covering, vinyl floor tile, carpet, and ceramic tile on a concrete substrate
Survey Limitations/
Inaccessible Areas
The fourth and fifth floors were occupied by an inpatient mental health clinic; although the remaining areas of the building were vacant, a large portion of the rooms were locked or used as storage space. Various areas of the vacant spaces had limited lighting and some areas had no lighting. Debris and remnant furnishings were observed in various vacant areas throughout the interior of the building. Hard ceilings were installed in various areas throughout the interior of the building.
General Observations: Conditioned air is provided to the building by multiple AHUs and rooftop air handler units (RTUs) that are located in various mechanical rooms throughout the building and on the roof. A central chiller tower is also located on the exterior of the building.
Air Handler Units:
Air Handler Unit (AHU) G19 was assessed in the main hospital basement. Based on information provided by CBRE personnel, this AHU is representative of a typical AHU for the main hospital.
The main ducts were wrapped with exterior insulation. Standing water was observed in the drain pan below the coils. Approximately 4 square feet of suspect fungal growth was observed on the metal walls of the AHU. No suspect fungal growth was visible in the supply plenum or visible duct run.
Rooms and Corridors:
Moisture damaged building materials were observed in various areas throughout the building. The moisture damage appeared to be related to historic rain water intrusion through the building envelope and/or historic leaks or condensate drip from plumbing fixtures and mechanical components.
Responsive ■ Resourceful ■ Reliable 6
Suspect visible fungal (mold) growth was observed in various areas throughout the building. The suspect fungal growth was observed on walls, ceilings, ceiling tiles and HVAC supply registers.
Surface samples were collected from a drywall wall in the fifth-floor elevator lobby, drywall wall in
Room 384 located on the third floor, drywall wall in the east nurse’s station on the second floor and metal HVAC supply register in the basement storage area. Microscopic analysis identified
Stachybotrys, Chaetomium, Cladosporium, and Memnoniella from the referenced sample locations
(Appendix A). The quantity of visible fungal growth observed in these areas was less than the regulatory threshold of 25 square feet as defined in the MARAR. Destructive sampling or disassembly of mechanical components was not within the scope of work for the project. Terracon did not access areas underneath floor tiles, wall paneling, or inside wall cavities. Fungal growth may be present in these areas, if moisture intrusion has occurred.

Interior Floors Unfinished concrete, floor coating, resilient floor covering, vinyl floor tile, raised flooring, carpet, and ceramic tile on a concrete substrate
Survey Limitations/
Inaccessible Areas
Multiple medical suites located in various area of the building were occupied and operational at the time of the survey. The tenants of the occupied suites did not allow unlimited access and access was not granted to the final suite until June 14, 2019. Although the remaining areas of the building were vacant, a large portion of the rooms were locked or used as storage space. Various areas of the vacant spaces had limited lighting and some areas had no lighting.
Debris and remnant furnishings were observed in various vacant areas throughout the interior of the building. Hard ceilings were installed in various areas throughout the interior of the building.

AHU P-05 was assessed on the third floor of the Plaza Building. Based on information provided by
CBRE personnel, this AHU is representative of a typical AHU for the Plaza Building. No access was available to the fan box, coils or supply air plenum, due to being bolted on. Suspect fungal growth was observed on the interior insulation of the AHU. The quantity of suspect visible fungal growth on interior insulation within the return air plenum was estimated at > 25 square feet, which exceeds the regulatory threshold of 25 square feet established in the Mold Assessors and
Remediators Administrative Rules (MRAR).
moisture damage appeared to be related to historic rain water intrusion through the building envelope suspect fungal growth was observed on walls, ceilings, ceiling tiles and doors/door frames. Surface samples were collected from a drywall wall in Suite 311, ceiling tile in a second-floor corridor closet, and drywall wall in Suite 203. Microscopic analysis identified Stachybotrys, Cladosporium, and
Ulocadium from the referenced sample locations (Appendix A). The quantity of visible fungal growth observed in these areas was less than the regulatory threshold of 25 square feet as defined in the
MARAR. Destructive sampling or disassembly of mechanical components was not within the scope of work for the project. Terracon did not access areas underneath floor tiles, wall paneling, or inside wall cavities. Fungal growth may be present in these areas, if moisture intrusion has occurred.
A surface sample was collected from the ceramic wall tile in the Day Surgery area located on the first floor. Microscopic analysis identified Cladosporium from the referenced sample location
(Appendix A). The quantity of visible fungal growth observed in the day surgery area and basement storage room were above the regulatory threshold of 25 square feet as defined in the MARAR.
