About this file

This is a reference questionnaire issued by the Nevada State Purchasing Division, Department of Administration, for Solicitation Number 90DETR-S3968 related to a Client Payment Card Program contract opportunity. The questionnaire is designed to collect client references from vendors and subcontractors proposing to provide services for projects of similar size and scope. Vendors are required to submit completed reference questionnaires to Joel Wixon, Purchasing Officer II, at j.wixon@admin.nv.gov by September 14, 2026, with the solicitation number included in the subject line. The State of Nevada may contact references for further clarification beyond the submitted questionnaire responses.

The reference questionnaire requires clients to evaluate vendor performance across thirteen rated criteria, including knowledge and expertise, flexibility relative to scope and timeline changes, quality of delivered materials and products, staff interaction and dynamics, schedule adherence and timeliness of deliverables, customer service responsiveness, billing accuracy, problem resolution capability, and likelihood of future recommendations. Ratings are provided on a scale of 0-10, with performance categories ranging from poor or inadequate (0) to exceptional performance (10). Clients are instructed to answer all questions with numeric ratings or explanatory responses; if information is unknown, responses should indicate "U/K," and if not applicable, "N/A." The questionnaire also requests narrative responses identifying the capacity in which the vendor was engaged and aspects of services the client is most satisfied with. Completed questionnaires are designated as confidential business information and are not subject to public disclosure under Nevada Revised Statutes.
