About this file

This is a Small Business Utilization Form for the City of Fort Worth, Texas, required for bidders on the PW Brick Pavement Rehabilitation Unit Price Contract. The City of Fort Worth has established a 30% small business utilization goal for this project. Bidders must complete this form in its entirety and submit it with their bid; failure to do so will render the bid non-responsive. All small business firms listed must be certified by an acceptable certifying agency—including the North Central Texas Regional Certification Agency (NCTRCA), Women's Business Council-Southwest (WBCS), Dallas/Fort Worth Minority Supplier Development Council (MSDC), or Texas Department of Transportation (TXDOT)—and must be located within the designated marketplace of Denton, Parker, Wise, Tarrant, and Johnson counties at the time of bid submission. The contract may be renewed up to two additional terms upon expiration of contract funds, with a maximum total contract duration of five years from the initial effective date.

Prime bidders must identify all subcontractors and suppliers regardless of certification status and provide complete documentation including business names as listed on certifications, addresses, contact information, NAICS codes, types of services or supplies, certification agencies, tier specifications, and dollar amounts for each firm. The form requires bidders to calculate and report the total dollar amounts for certified prime bidder services, small business subcontractors and suppliers, non-small business subcontractors and suppliers, and the overall total contract value. Prime bidders are prohibited from making additions, deletions, or substitutions to the certified list without prior City approval, and any unjustified changes constitute material breach of contract and may result in debarment. Bidders must also consent to provide audits and examinations of books, records, and files, as well as authorize interviews with owners, principals, and applicable subcontractors to substantiate work performed by small business firms. Intentional or knowing misrepresentation of material facts results in debarment for a minimum of one year.
