About this file

This is a Pre-Employment or Pre-Service Affidavit form required by Richardson Independent School District (RISD) under Texas Education Code Section 22A.055 for all individuals applying for employment or service provider positions with the educational entity. The form mandates disclosure of work history with educational institutions, consent for release of prior employment records, and any investigations, charges, convictions, or licensing actions related to student abuse, inappropriate relationships, improper communications, or boundary violations as defined by TEC 22A.051. Applicants must also disclose terminations, non-renewals, discharges, or resignations from educational institutions and any current subject status in investigations or disciplinary actions by schools, licensing agencies, law enforcement, or courts. The form applies to all service types supporting RISD operations, including those related to the district's $1.4 billion bond program for construction, renovations, infrastructure upgrades, and facility improvements.

Failure to disclose required information constitutes a Class B misdemeanor and grounds for termination of employment or service. Applicants must acknowledge the truthfulness of their disclosures as of the signature date and affirm a continuing duty to update the affidavit immediately if any changes occur in employment status, investigations, charges, convictions, licensing actions, or other required disclosures. Completed forms must be submitted to generalinquiry@tea.texas.gov or, if submitted after a bid award, to purchasing@risd.org. The affidavit form requires applicants to provide personal identifying information, including name, date of birth, address, county, associated vendor name, RISD bid number or cooperative contract number, and service type description.
