About this file

This is a RRC Contract Facility Certification of Compliance form template used in conjunction with a Request for Proposals (RFP).

The form serves as an attestation by a signatory that a specified facility meets all applicable compliance requirements. The signatory must certify that the facility identified by name, address, and telephone number complies with the most current local, state, and federal laws and regulations—or whichever standard is most stringent—regarding facility safety, zoning, occupancy, Architectural Barriers Act (ABA) compliance or reasonable accommodations provided off-site, and air exchange specifications. The certification references compliance standards outlined in both the Statement of Work and the associated RFP solicitation. The form requires the signatory to provide their printed name, title, signature, and date of certification. This document is a template requiring completion for submission as part of an RFP response.
