About this file

This is a Certification of Bidder Regarding Equal Employment Opportunity form required for the State Theatre External Rehabilitation project, a sealed bid invitation issued by the Redevelopment Authority of the City of Uniontown, Pennsylvania. The certification form requires bidders to disclose their participation in previous contracts subject to the Equal Employment Opportunity Clause under Executive Order 11246. Sealed bids for Contract No. C000083242 must be submitted electronically via PennBid by the specified deadline on Wednesday, January 7th, 2026, with bids to be reviewed by 4:00 PM the same day at 20 North Gallatin Ave, Suite 216, Uniontown, PA 15401. A non-mandatory pre-bid conference will be held at the project site on December 22nd, 2025, at 11:00 AM. Each proposal must be accompanied by a bidder's bond or certified/cashier's check equal to not less than 10% of the bid amount, and bidders must submit a non-collusion affidavit with their bids.

The certification form requires bidders to state whether they have participated in previous Equal Employment Opportunity Clause contracts, whether compliance reports were required, and whether all compliance reports including SF-100 forms have been filed. If the certification indicates that required compliance reports have not been filed, the bidder must submit such reports within seven calendar days after bid opening, or no contract award will be made. Bidders must also disclose whether they have been or are being considered for sanctions due to violations of Executive Order 11246. The project is funded through the Community Development Block Grant Program and is subject to all related federal regulations, including federal prevailing wage requirements. No bid may be withdrawn for sixty days after submission.
