The form is required for vendors seeking to enter contracts valued over $50,000 and must be notarized and submitted with a bid, proposal, or offer. The document requires vendors to disclose detailed financial interests, potential conflicts of interest, and relationships with HCC employees or other government agencies.
The disclosure form comprehensively covers four key sections: (1) Financial Interests in the Vendor, which requires reporting of ownership percentages, income shares, and property interests; (2) Potential Conflicts of Interest, including current and previous employment relationships; (3) Gifts received by the vendor or their immediate family members; and (4) Other Contract and Procurement Related Information, such as current and pending contracts with other Texas government agencies. Vendors must certify the accuracy of their disclosure and understand that failure to provide complete information may result in bid rejection or potential prosecution. The form emphasizes ongoing disclosure obligations, requiring vendors to submit updated information within seven days of discovering significant changes in financial interests.
