About this file

This document outlines the mandatory minimum requirements and technical scoring criteria for the MK27-105 WIC Pharmacy Services contract opportunity issued by the State of Utah Department of Health and Human Services Division of Purchasing. The RFP seeks a single vendor to provide pharmacy services for the Women, Infants and Children (WIC) program, with a contract term of five years. All orders must be fulfilled by a Registered Pharmacist or Doctor of Pharmacy, and the vendor must maintain a stocking location within Utah with medical foods and formulas currently in inventory to ensure rapid availability. The vendor must also demonstrate the capacity to deliver to all facilities identified in the Delivery Locations document and provide comprehensive pricing inclusive of shipping and handling for all items listed in the cost sheet, covering both the 30-item market basket and required products.

To advance in the procurement process, vendors must satisfy all four mandatory minimum requirements, which constitute a pass/fail evaluation stage. Upon passing the mandatory minimums, vendors will be scored on five technical criteria on a 0-5 scale: scope of work alignment, timely delivery plan, demonstrated experience (requiring a minimum of two years of successful similar service delivery), facility quality control protocols, and professional references (at least one completed reference form). Vendors must achieve the minimum technical score threshold specified in the RFP to progress to the cost evaluation stage. Pricing must be guaranteed for one year, and the contract award will be made to the responsive and responsible vendor with the highest overall score as justified by procurement code.
