About this file

This is an RFP evaluation scoresheet for Solicitation #MK27-105 WIC Pharmacy Services, issued by the State of Utah Division of Purchasing on behalf of the Department of Health and Human Services. The RFP seeks to establish a single contract award with a responsive and responsible vendor to provide pharmacy services for the Women, Infants and Children (WIC) program. The contract term is five years, with vendors required to guarantee pricing for a minimum of one year.

The evaluation scoresheet establishes a comprehensive scoring methodology allocating 725 total points to technical criteria and 310.7 points to cost evaluation. Technical evaluation criteria include Scope of Work Alignment (250 points, 34.5%), Timely Delivery Plan (250 points, 34.5%), Demonstrated Experience (100 points, 13.8%), Facility Quality Control (100 points, 13.8%), and Professional References (25 points, 3.4%). A minimum technical score threshold of 65% (471.3 points) is required for evaluation consideration. Four mandatory requirements must be satisfied on a pass/fail basis: Certification and Licensing, Cost Sheet submission, Delivery Coverage, and Utah-Based Inventory. Cost points are calculated using a comparative formula in which the lowest proposed price receives 100% of the 310.7 cost points, with all other proposals receiving proportionally reduced points based on their price variance from the lowest bid. Total evaluation points are distributed as 70% technical (725 points) and 30% cost (310.7 points), yielding a maximum possible score of 1,035.7 points.
