About this file

This is a Cost Proposal pricing sheet for cafeteria furniture, installation, and related products and services, submitted as Attachment 4 to a Master Cooperative Purchasing Agreement being established by the West Ada School District (WASD), the largest school district in Idaho with approximately 38,000 students across 60 school sites. The agreement will be available to current and future members of Equalis Group nationwide and will be funded in whole or in part by USDA federal funds in compliance with Idaho Code, 2 CFR Part 200, and applicable USDA regulations. The solicitation seeks qualified organizations to provide competitively priced cafeteria furniture and installation services, with award to one or more vendors determined to be in the District's best interest.

Offerors must submit pricing using a discount-based model, providing a percentage discount off manufacturer list prices for all offered products and identifying the price list location to verify compliance. Freight costs must be expressed as a percentage of net product cost (after discount applied) and must either be included in the catalog price with a $0 entry or priced separately, but not bundled. Installation pricing shall be provided as a percentage of net product cost across five service categories: Off the Truck, Cased Goods Installation, Panel Systems Installation, Industrial Lift equipment usage, and White Glove service. All percentages provided represent maximum ceiling rates for freight and installation charges. Offerors may add lines as necessary to capture multiple categories and manufacturers.

ATTACHMENT 4: COST PROPOSAL
Cafeteria Furniture, Installation and Related Products & Services
PRODUCTS, SERVICES, AND SOLUTIONS PRICEBOOK
Instructions: Offerors shall use this template to provide a Percentage Discount Off Manufacturer List Price for offered products. Offerors may add lines as necessary to capture multiple categories/manufacturers as needed. Freight: If freight costs are included in the product catalog price, they must not be listed or priced separately.

In this case, the Offeror shall enter “$0” in the freight pricing table to reflect no charge.

If freight costs are not included in the product catalog price, they must be priced separately and may not be bundled in the Discount‑off‑List price.

Freight pricing shall be expressed as a percentage of the product’s net cost, defined as the product cost after the Discount‑off‑List has been applied.

Installation: Offerors shall provide a % of Net Product Cost per category that will be charged for Installation. Category Definitions: 1. Off the Truck A basic delivery service where product is simply unloaded from the truck to the customer’s dock or designated receiving area without installation, assembly, placement, or packaging removal.

2. Cased Goods Installation Installation service for fully assembled or minimally assembled furniture, typically individual items that do not require complex field assembly.

3. Panel Systems Installation of modular workstation systems (cubicles or benching systems) that require professional assembly, electrical integration, and system alignment.

4. Industrial Lift (or Using Lift Equipment) Additional installation cost associated with using mechanical lifting equipment to move or position heavy, oversized, or high‑elevation furniture or components. 5. White Glove Service refers to a premium, end‑to‑end furniture installation offering that goes beyond standard drop‑off. It typically includes:

Inside Delivery: Items are brought into the building and transported to the designated room or floor.

Unpacking: All packaging materials are removed from the items.

Assembly & Installation: Furniture is fully assembled, installed, leveled, and placed according to the floor plan or customer instructions.

Quality Check: Installers verify stability, correct placement, and product condition.

Debris Removal: All boxes, pallets, and packing materials are removed from the site.

Careful Handling: Enhanced care to avoid damage to the furniture, floors, walls, and surrounding areas. The percentages provided shall represent the maximum ceiling rate for Freight & Installation charges.
