About this file
This file is a past performance survey template and example documentation used to evaluate contractor performance on previous federal contracts. The document contains two components: a completed past performance information sheet showing an example from the HUD Office of Public and Indian Housing with a technical assistance contract (C-DEN-12345, valued at $1,000,000, performed January 2007 through January 2009), and a blank past performance survey form to be completed by references.
The survey template requires references to evaluate contractors or key personnel across five rating categories: Quality of Service, Schedule, Cost Control, Business Relations, and Management of Key Personnel. Each category uses a five-point adjectival rating scale ranging from Exceptional to Unsatisfactory, with a Not Applicable option available for Cost Control. References must provide substantive comments for any ratings marked as Exceptional, Very Good, Marginal, or Unsatisfactory. The survey is designed to be completed by government clients or points of contact who oversaw the contractor's work and can attest to performance on specific projects, contracts, or key personnel assignments. This template is used as part of HUD's proposal evaluation process for construction inspection services and other contract solicitations where past performance is a significant evaluation criterion.
