1) Replace paragraph 3 of DRD No. REMIS2-PM-01 with the following "Describe the quality processes that will be used for ISS systems flight hardware development and the quality processes used to implement payload hardware/software development and MI&O tasks."
2) Update DRD No. REMIS2-PR-02 to exclude the Fringe Benefit Table from the page count.

8. PREPARATION INFORMATION: The contractor shall prepare the deliverable as follows:
SCOPE
The Program Management Plan (PMP) shall describe the Contractor’s organizational structure, management approach, and processes and practices that will be used to deliver (as applicable) spaceflight hardware and software, sustaining engineering functions, engineering services, payload facility integration, and/or research Mission Integration and Operations (MI&O) services defined in the Statement of Work (SOW).
CONTENT
A. MANAGEMENT: Address the Contractor’s plan for work definition and authorization, scheduling, budgeting, cost data accumulation and reporting, safety and quality assurance, subcontracting, material control, indirect cost management, baseline control, knowledge capture, and organization structure and culture, including the following:
1. Describe the risk management approach and explain how risk management is implemented in the day-to-day management of the contract. Describe how risks (management and technical) are identified, monitored, and controlled.
2. Provide a contract specific organization chart, including subcontractors and teaming partner[s], if proposed, and rationale for the approach selected. If a subcontracting or teaming arrangement is proposed, include specific details on how the subcontractor or teaming partner shall be managed, how they will be incentivized, the business size of each company, the relative amount of work to be performed by each, and if there are discreet tasks to be performed by each or whether there is comingling of personnel and resources, division of tasks and responsibilities relative to the contract SOW. Describe how teaming arrangements will on-ramp when deemed necessary to compete for task orders under this contract.
3. Describe how an efficient and effective interface to the Government will be implemented to support the performance, management, communication, and completion of the SOW requirements, contract schedules, and deliverables.

4. Describe the requirements, responsibilities, and procedures for meeting Configuration Management (CM) requirements per SSP 41170, Configuration Management Requirements, as applied to this contract.
5. Describe the certification methodology to be used that meets ISS Certification of Flight Readiness (CoFR) requirements.
6. Describe the implementation approach (how, where, capabilities) for supporting payload research and operations during ground and on-orbit operations.

10. FIRST SUBMISSION DATE: Plan shall be submitted in draft form with the proposal and submitted for final approval forty-five (45) calendar days after contract award.
Frequency of Submission: After initial approval - the plan shall be reviewed annually and submitted if updates are required to reflect changes. If no changes to the plan are required, the Contractor shall submit a delivery notice indicating this.
Additional Submissions: As requested by CO.

8. PREPARATION INFORMATION: The contractor shall prepare the deliverable as follows:
SCOPE:
The Total Compensation Plan (TCP) shall identify and discuss wages, salaries, and fringe benefits for professional employees and non-exempt service employees in all proposed labor categories, including those personnel subject to union agreements, the Service Contract Labor Standards, and those exempt from both of these. By evaluating the compensation of all labor categories, this plan combines the requirements of FAR 52.222-46 and JPI 1852.231-71. The Plan shall address the requirements of the Service Contract Labor Standards and commit to the compliance of all wage determinations.
The Total Compensation Plan and Fringe Benefits Analysis of Compensation Plan (FBACP) template shall be required for the prime team members when the total potential value is expected to exceed the certified cost or pricing data threshold set forth in FAR 15.403-4 and for all subcontractors that meet the criteria in JPI 1852.231-71(d). The FBACP template shall be provided as part of the Cost/Price Volume of the proposal and will be evaluated as part of the Total Compensation Plan. The TCP should be consistent with this template. (The FBACP template is not required as part of a TCP submitted after award.) For hybrid contracts: A TCP is not required for the subcontractor(s) that provide(s) support for only the fixed price portion of this procurement. If the subcontractor(s) provide(s) support for the fixed price and the cost reimburseable portion, a TCP is required. The TCP (excluding the templates) shall become part of the contract as Attachment J-8. A compliance matrix is not required. For TCPs submitted after award, the contractor shall submit either a Change Matrix or submit a copy of the TCP in track changes mode.

B. The contractor/team shall describe their commitment for compliance with the Service Contract Labor Standards, any other applicable laws, regulations, statutes, and guidance and all wage determinations, which includes any applicable collective bargaining agreements.
C. The contractor shall include the rationale for any conformance procedures used or those Service Contract Labor Standards employees proposed that do not fall within the scope of any classification listed in the applicable wage determination.
D. State contract year one direct labor rates and planned labor rate escalations for exempt and non-exempt employees. Explain how wage/salary ranges and their corresponding escalation were established for each labor classification identified. Supporting information shall include data, such as recognized national and regional compensation surveys and studies of professional, public, and private organizations used in establishing this proposed TCP. If the offeror is relying upon Wage Determination updates to increase the wages of contract non-exempt and non-union personnel, explain how your company plans to retain these personnel and maintain morale should the Department of Labor not escalate their wages. The contractor shall provide written support to demonstrate that its proposed compensation is reasonable. To demonstrate evidence of reasonableness, the contractor shall provide the applicable portion of the surveys/ studies. The surveys/studies provided are not part of the page count.
E. If uncompensated overtime is proposed, it shall be in accordance with FAR 52.237-10, “Identification of Uncompensated Overtime”. If proposed, the contractor shall discuss the effects of uncompensated overtime on the Total Compensation Plan and provide a discussion as to whether the uncompensated overtime is voluntary or involuntary. Describe the possible effects that uncompensated overtime will have on employee morale and retention. The contractor shall provide a copy of the company policy for uncompensated overtime with proposal. This uncompensated overtime policy is not part of the page count.
F. State the policies on seniority for benefits with regard to exempt employees. For example, with regard to leave and retirement programs, from what date is seniority calculated – the date of hire, service on the prior NASA contract, or service on all prior NASA contracts?
G. Discuss the contractor’s company’s fringe benefit policies and practices. Each Offeror must propose a compensation structure that is sufficient to recruit and retain employees.
• For each benefit, discuss whether the benefit applies to exempt employees, non-exempt and non-union employees, and/or non-exempt and union employees, (or some combination thereof).
• For Vacation (or Paid-Time Off) map the hours to the years of service. For example: 1-5 years = X hours; 5-10 years = X hours; 10 -15 years = X hours, 15-20 years = X hours, and 20+=X hours.
• For insurance benefits, discuss whether the benefit applies to the employee only, or the employee and family.
• If a category is non-applicable, then insert: “N/A” in the box.
• Each item must be addressed. Offerors may add fringe benefit categories to the table below but cannot delete benefit categories. The table may be modified to convey the information (Table is on the next page)
CONTENT:
A. State the company name(s) of the prime Offeror (or joint venture team members) and subcontractor(s), using the subcontractor definition found at JPI 1852.231-71(b). For subcontractors that will submit a TCP, state whether the Government can share your compensation data with the prime Offeror.

The page limit for the TCP is 15 pages for the Prime and 15 pages for each subcontractor [subcontractors are defined in JPI 1852.231-71(d)]. For joint ventures, the page limit is 15 pages per team member. The prime, subcontractors, and team members are permitted to combine data into one TCP, and the page limit for the combined TCP will be 15 pages. The Fringe Benefit Table is not included in the overall TCP page limit.

8. PREPARATION INFORMATION: The contractor shall prepare the deliverable as follows:
SCOPE: The Government Property Management Plan defines the Contractor's use, maintenance, repair, protection, and preservation of Government personal property. It shall describe the Contractor's approach to receiving, handling, stocking, maintaining, protecting, and issuing Government property (equipment and material). The Plan should include interaction and Department/ Office responsibilities. The Contractor shall submit to the delegated Government Property Administrator (GPA) detailed supplemental property procedures, which are separate from the property management plan, within 60 days after the contract start date.
In addition to submitting an initial Government Property Management Plan, the Contractor shall also submit proposed property manager qualifications and experience as specified below.

Management Qualifications/Experience:
A. The Offeror/Contractor shall specify the name and the qualifications of the proposed property manager.
1) List the Federal agencies or departments supported in managing Government property and the corresponding number of years of experience.
2) List completed personal property management training courses.
3) Specify the level of professional property management certification obtained by the proposed property manager.
4) List professional personal property management organizations in which the proposed property manager has an active current membership.
B. This qualification data shall be a one-time submittal from the Offerors with their initial Government Property Management Plan. Insert this data as a separate tab after the property management plan.

L.24 COST AND PRICE FACTOR – VOLUME III
L.24.1 COST AND PRICE PROPOSAL INSTRUCTIONS
There is no page limit for the cost and price volume. The intention of this solicitation is to obtain the work described herein by means of a contract with an Indefinite Quantity Task Order effort. The contract will be for a 7-year basic effort and one 2-year option period. For pricing purposes, the Offeror shall use the total potential ordering period from October 1, 2023, to September 30, 2032.
The Government assumes that adequate price competition exists and that the amount and types of data required will be adequate to ensure the cost/price proposals have been prepared on a sound basis that will allow the Government to evaluate the Offeror’s competitive approach, and to determine cost realism. Therefore, the submission of certified cost or pricing data is negated (FAR 15.403-1) and will not be requested unless proposed prices appear unreasonable or unrealistically low and there are concerns that the Contractor may default; and/or if only one proposal is received in response to the solicitation (FAR 15.403-1(c)). Offerors will be advised if additional ‘other than certified cost or pricing data’ is required (FAR 15.403-3).
Should the Government deem it necessary, comprehensive audits of the prime and any of the subcontractor’s proposals may be performed by another Government Agency, such as the Defense Contract Audit Agency (DCAA), or an independent public accounting firm (pursuant to NFS 1815.404-2(a)(1)(F) “Use of Contractor to Perform Contract Audit Services”). These audits may include “business system audits” such as audits of the Offeror’s accounting, estimating, purchasing, compensation, and/or other internal controls. If awarded the contemplated contract, the Offeror is expected to have or obtain the proper accounting/estimating systems for Cost- Reimbursement type contracts in accordance with FAR regulations.
The cost/price proposal should encompass all costs associated with the requirements of this solicitation and shall comply with applicable Federal Acquisition Regulation (FAR), NASA FAR Supplement (NFS), and governing statutory requirements.
The Cost and Price volume for the Offeror should consist of three Parts, with each Part consisting of various Sections. Each Part and Section should be clearly labeled. Table L-3 outlines the structure of the three Parts and their respective Sections.

SECTION 1: COST/PRICE PROPOSAL INTRODUCTORY PAGE AND TABLE OF
CONTENTS
The following information is required on the introductory page of the cost/price proposal:
• Title: “Volume III: Cost/Price Proposal”
• Solicitation Number
• The Offeror’s (or Business) name and address
• CAGE Code
• Type of contract, place(s), and period(s) of performance
• The total proposed amount, stated in Cost, Fee, and Total
• Name, address, telephone, and fax number of the Government cognizant contract audit office
• Name, address, telephone, and fax number of the Government cognizant contract administration office
• Name, title, telephone number, and e-mail address of a Point of Contact familiar with and authorized to provide additional information concerning the Cost Proposal to both NASA and Auditors from other Government agencies
The cost and price volume should include a table of contents for ready reference to key parts, figures, and illustrations. For convenience, the cost and price volume may be divided into separate files, provided they are properly identified as such, e.g., “Volume III, Part I”, and must adhere to all other format and page-limitation instructions given herein. For Part 2, each of the workbooks should be clearly labeled. Cost and Price Volume, Part 3 should be in a separate file for ease of use during proposal evaluation.

SECTION 2: SUBCONTRACT COST ANALYSIS
Per FAR 15.404-3(b), Offeror and any subcontractors are instructed to “conduct appropriate cost or price analyses to establish the reasonableness of proposed subcontract prices” and “include the results of these analyses in the proposal”. The proposal should provide details and a discussion on all adjustments made to the subcontractor’s rates, including any adjustments on fee/profit.
The proposal should provide a discussion on the use or non-use of any adjustments based on Offeror history with the subcontractor.
SECTION 3: ESCALATION
Offeror should include all escalation (i.e., Direct Labor Rate, Indirect Rate) in the submitted cost/price proposal. Offerors are required to propose realistic direct-labor escalation rates.
Offerors should provide adequate documentation in support of all proposed direct-labor rates.
Wage/salary increases should follow any applicable union agreements, collective bargaining agreements, wage determinations, etc. Offerors should provide the most recent three years of historical labor escalation for similar projects, if available. Include the rationale and methodology used for the annual escalation rate development – including escalation assumptions, sources of projections, how these rates reflect your prior company experience, and how they relate to your total compensation package.

SECTION 4: WORKBOOKS
Specific Instructions:
Formulas: All formulas used in the electronic spreadsheets must be clearly visible. The workbooks should not contain macros or hidden cells. The use of external links is prohibited.
Pre-populated Formulas: Selected templates have formulas pre-populated by the Government as a courtesy. Offeror is responsible for ensuring the accuracy of these formulas and editing/correcting them as necessary.
Locks: The electronic submittal should not be locked/protected or secured by passwords in part or in whole.
Scanned Documents: The Government has provided Cost/Price templates for completion by the Offeror in a Microsoft Excel format. Offeror is instructed to submit the templates in a Microsoft Excel format and should not submit scanned versions in the electronic submittal.

Rounding: Monetary amounts, other than Direct Labor Rates, may be expressed to the closest whole dollar amount, with cents omitted. Indirect Rate Percentages and Direct Labor Rates should be expressed to no less than the second decimal place.
Offeror is instructed to complete all templates provided in the attached Microsoft Excel Workbook, REMIS2 Cost Price Summary Template.xlsx, Attachment L-5. The Offeror may resize/edit the templates to better fit the proposal package and/or its estimating and accounting practices but must adhere to the format designs provided. The Offeror’s own format should be used for supporting information.
Template 1: Cost-Price Summary
This template is designed to summarize the total cost and price by year from the Cost-Plus Fixed Fee (Template 2) and Firm-Fixed Price (Template 4) templates.
Template 2: CPFF IDIQ Labor Rates
The Government may order services on an IDIQ basis at any time during the base period and option years.
Prime Offerors are required to submit a schedule of not-to-exceed cost-plus fixed fee fully burdened straight time hourly rates by year.
Fully burdened productive straight time labor rates are required for each labor category applicable to the RFP (reference contract attachment J-4, Standard Labor Categories*). The fully burdened labor rates should include wage, salary, health and welfare cost, overhead, G&A, and escalation for all personnel. The total cost will consist of one hour of labor for each standard labor category that was provided with fee applied. The total price of all standard labor categories will be used for evaluation purposes. Any additional labor categories proposed will be evaluated for reasonableness.
* Offeror may propose additional labor categories but must fill-in the proposed rates in the cost and price worksheets and the rate table in contract section B (clause B.9, “CPFF IDIQ Rates”) and provide the proposed labor category’s description and qualifications in Attachment J-4, Standard Labor Categories.
Template 3: CPFF IDIQ Rate Development
This template is the supporting work sheet for the “CPFF IDIQ Labor Rates Template.” Prime Offerors are required to submit the IDIQ labor rate development template for each year in the seven-year base ordering period and the two-year option ordering period. Additionally, the Offeror shall map their respective labor categories to the Government provided standard labor categories.
Offerors shall assume the government will not award any REMIS2 Task Orders when forecasting their business base for the development of labor, non-labor, and burden rates for this effort.
Column H shall correspond to Section B.9.

Template 5: FFP IDIQ Rate Development
This template is the supporting work sheet for the “FFP IDIQ Labor Rates Template.” Prime Offerors are required to submit the IDIQ labor rate development template for each year in the seven-year base ordering period and the two-year option ordering period. Additionally, the Offeror shall map their respective labor categories to the Government provided standard labor categories.
Offerors shall assume the government will not award the IDIQ effort when forecasting their business base for the development of labor, non-labor, and burden rates for this effort.
Column H shall correspond to Section B.8.
Templates 6a, 6b, and 6c: Indirect Rate
This template is designed to collect information pertaining to the indirect rates utilized in the proposal and should be duplicated as necessary.
Offerors will utilize established Forward Pricing Rate Agreement (FPRA) or Forward Pricing Rate Recommendation (FPRR) data in the development of their cost proposal, if applicable. For all FPRAs and FPRRs utilized in an Offeror’s cost proposal, the Offeror is to provide a statement identifying the agreement by report number, date issued, and the issuing agency’s office and
L-31 phone number. A copy of the FPRA and/or FPRR is to be included (this document may be a scanned PDF file). Should an Offeror deviate from the published FPRA or FPRR, a written explanation and justification shall be included in the supporting information, stating the rationale and methodology used for the varying rate development and indicating a clear description of the projected rate. Offerors utilizing a FPRA for indirect rates are instructed to not complete and submit an “Indirect Rate” worksheet for that FPRA rate.
Absent a FPRA/ FPRR, the Offeror is instructed to submit a template for each indirect rate proposed (i.e., labor overhead, fringe benefits, general & administrative) and should list the cost elements of each pool and the dollar amount of each cost element by Contractor Fiscal Year.
Offeror should include the dollar amount of the application base and calculate the resultant rates.
In the event the Offeror’s fiscal year and anticipated contract year do not coincide, the Offeror is instructed to reconcile the rate and explain how the fiscal-year rates result in the proposed contract-year rates. Offeror is instructed to also indicate the rate for the previous three years in the appropriate location and disclose the application base at the top of the template. Offeror is instructed to include a narrative discussion supporting estimates and explaining significant changes in either expenses or the application base that exists from one year to the next.
Template 7: Cognizant Audit Office Template (CAOT)
This template is designed to capture relevant information concerning (1) the specific location (address or addresses for prime and proposed subcontractors, as appropriate) where auditable cost information that supports amounts proposed physically resides; (2) the person or persons (name, address, phone number, and e-mail address) who can be contacted by DCAA to provide audit information for the prime Offeror, (3) the person or persons (name, address, phone number, and e-mail address) who can be contacted by DCAA to provide audit information for companies, partners (in a teaming, joint venture, or partnership situation) or proposed subcontractor(s); and
(4) the name and address of the cognizant DCAA field-audit office to which electronic and hardcopy proposals were sent. The name, location, and contact information for the DCMA DACO or ACO.
Template 8: Fringe Benefits Analysis of Compensation Plan (FBACP)
Refer to Section J-1, Data Requirement Description REMIS2-PR-02, Total Compensation Plan, for instructions pertaining to this template.
COST AND PRICE VOLUME – PART 3: CONTRACTOR BASIS OF ESTIMATE

SECTION 5: COST NARRATIVE/ CONTRACTOR BASIS OF ESTIMATE (BOE)
Offeror is instructed to provide narrative support of all proposed estimates. The narrative should explain in detail: pricing and estimating techniques, the basis of all projections (i.e., rationale, L-32 assumptions, and cost methodologies), cost estimating relationships, and judgmental factors of cost projections. The Offeror’s own format may be utilized.

L.25 RESPONSIBILITY CONSIDERATIONS – VOLUME IV
Additional information is required to determine responsibility in accordance with FAR 9.104-1, General Standards as stated below.
(a) Responsibility Information
1) With Initial Proposal Submission: Offerors shall include Responsibility Determination information (per FAR 9.104, Standards) that includes their:
a) Organizational Conflicts of Interest (OCI) information: In accordance with FAR Subpart 9.5, Organizational and Consultant Conflicts of Interest, the offeror and proposal will be reviewed for existing and potential OCI issues in relation to this procurement. The offeror shall submit the information required by the OCI Plan
DRD-REMIS2-PR-03.
b) Responsibility Determination Disclosures (Financial Capability): To determine if an Offeror is in accordance with FAR 9.104-1(a), Offeror is instructed to submit information which demonstrates its financial capability to perform the contract. Acceptable information includes letters from certified United States banks indicating the available amount of credit for the business and the company’s annual report. If a teaming arrangement, joint venture, or other business combination is contemplated, disclose each participant’s responsibility for financial management of the venture, funding requirements, limitation of liabilities, and any other information which describes the financial arrangement.
c) FAR 16.301-3, Cost-Reimbursable Contracts, Limitations: FAR 16.301-3, Cost-Reimbursable Contracts, Limitations requires that a contractor's accounting system be adequate for determining costs applicable to the contract prior to the award of a cost-reimbursement contract. This also applies to any subcontractor that is proposed to receive a cost-reimbursement type contract. Offerors and their proposed subcontractors shall provide evidence of an adequate accounting system as determined by the cognizant administrative office for accumulating and reporting incurred costs. If a determination of adequacy of your accounting system has not been made by the Government (normally DCAA), then the Offeror shall complete the “Pre-award Survey of Prospective Contractor Accounting System Checklist” found in Attachment L-6 and provide a copy of that signed document with Volume IV, Responsibility Considerations. An adequate accounting system is not an evaluation
L-33 criterion. It is a basic contract requirement with a pass/fail determination. Only offeror(s) with an adequate accounting system will be eligible for CPFF Task Orders.
A contract may be awarded to the offeror(s) who are not determined to have an adequate accounting system, however the offeror will only be eligible for FFP Task Orders.
Offerors are to provide for their respective companies:
(1) the current status of, (2) date of last review, and (3) name, address, and phone number of the governmental agency that performed the review for the business systems, and (4) the audit report number. If the review has not been performed or is not required, Offerors are required to provide a negative response. In addition to any narrative provided, Attachment L-7, Business Systems Reviews and Status Information Template.xlsx should be used to provide the required information. The attachment may be resized or modified by the Offeror, but the information provided should adhere to the basic format contained in the attachment.
d) Cost Accounting Standards: State whether the Cost Accounting Standards (CAS) Disclosure Statement represented in Provision K.7, FAR 52.230-1 Cost Accounting Standards Notices and Certifications, has been approved by the cognizant Administrative Contracting Officer, and provide the date of such approval. If your CAS Disclosure Statement is currently not approved or there are some existing CAS non-compliance findings, please provide detailed explanation of the CAS non-compliance issues, corrective action status, and any potential impact on this procurement. A copy of the offeror’s disclosure statement applicable to the resultant contract shall be submitted and the offer shall clearly identify where in the disclosure statement the following are addressed: Contractor Purchasing, Requirements for Cost Tracking and Accounting and Associated Business Systems.
e) Prime/Subcontractor Identification and SOW Division of Work Listing (Attachment L-8): The offeror shall provide a summary listing (by name and address) of all subcontractors (regardless of dollar value) that have been identified throughout the offeror’s proposal, the subcontract value associated with each entity, and the SOW sections that each subcontractor proposes to support by filling out Attachment L-8. This listing shall also include the subcontractors size classification (i.e., Small Business (SB), Woman Owned Small Business (WOSB), etc.) and cage code. Fill in so all SOW sections have a Prime and/or subcontractor associated with accomplishing that requirement.
f) Subcontracting Arrangement Information [for small business offerors only]: If a subcontracting arrangement is proposed, offerors shall include the following specific detail in the following areas so that the Government can determine that the prime contractor making the offer will be performing the primary and vital requirements for the contract.

• Who will manage the contract?
• Which party possesses the background and expertise necessary for contract performance?
• Which party pursued the contract?
• The degree of collaboration in preparation and submission of competitive proposal.
• Whether there are discreet tasks to be performed by each of the teaming partners, or whether there is, instead, commingling of personnel and resources.
• The relative amount of work to be performed by each teaming partner or subcontractor.
• Which party will perform the more complex and costly contract functions?
• The business size of each of the other parties.
• Which of the parties possess the qualifications relevant to the contract requirements?
• Describe how the offeror will ensure compliance with FAR 52.219-14, Limitations on Subcontracting. For the purposes of this RFP, JSC defines “costs of contract performance incurred for personnel” to mean direct labor, labor overhead (including payroll taxes and benefits), and G&A. “Employees of the concern” means actual direct employees identified on the Offeror’s payroll records for which social security taxes are matched. JSC considers “cost of contract performance” to include all costs throughout contract completion (the full contract period of performance).
g) Company Information: Provide the following information for the prime/joint venture partner, and each subcontractor with a value of $10 million and over:
• The company point of contact’s name, job title, phone number, email address, and physical address.
• The company business size, type, UEI number, and cage code.
2) With Final Proposal Revision (FPR) or from the Apparent Awardee(s) (if no discussions are necessary)
The remaining Responsibility Determination information addressing all of the rest of the elements under FAR 9.104, Standards, to demonstrate responsibility shall be included in the FPR. If no discussions are necessary, the information will be requested from the apparent successful offeror(s) pursuant to FAR 9.105-1. The following information will only be required for all Offerors who enter into Competitive Range or are the apparent successful Offeror(s).
Business System Adequacy Updates since the Initial Proposal Submission - When submitting the remaining Responsibility Determination information identified in FAR 9.104, state whether all business systems, including but not limited to accounting, property control, purchasing, estimating, project reporting and employee compensation, which require Government acceptance or approval (as applicable) are currently accepted/approved, by DCAA, without condition.
a) Provide documentation showing approval status including the following:

• Name of system/software.
• Length of approval, if applicable.
• Provide up to 3 Contract Name(s) and Numbers for which the business systems are currently in use.
• Provide the name, number and email address of the COR and CO, for each Government contract.
• Provide the date of acceptance/approval for each system. Explain any existing conditional acceptances/approvals and the compliance status of any systems(s) for which acceptance or approval is currently withheld.
b) Taxpayer Identification Number: Prime offerors shall provide their Taxpayer Identification Number (TIN) (the number required by the Internal Revenue Service (IRS) to be used by the Offeror in reporting income tax and other returns).
c) Waiver of Rights to Inventions: This solicitation contains NASA FAR Supplement (NFS) Clause 1852.227-70, “New Technology” and NFS provision 1852.227-71, “Request for Waiver to Rights to Inventions”. Any petitions for advance (prior to contract execution) waiver of rights to inventions should be included in this volume.:
d) Information Technology (IT) Systems Information: In accordance with NFS 1852.239-73, the Contracting Officer will ask the Apparent Successful Offeror to provide the following information for all IT systems offered:
(1) A brief description of the item(s).
(2) Vendor/manufacturer’s company name and address.
(3) If known, manufacturer’s web site, and the Commercial and Government Entity (CAGE) code.
The Apparent Successful Offeror(s) will be notified by the Contracting Officer of this requirement, with an anticipated three (3) days allowed for submittal.
The Senior Agency Information Security Official (SAISO), HQ Office of the Chief Information Officer IT Security Division, will review the IT systems offered. The Contracting Officer will advise the Offeror if any of its proposed IT systems are not approved and may provide the Offeror an opportunity to revise its proposal accordingly. The CO shall not make an award unless the SAISO, HQ OCIO IT Security Division, has reviewed and approved the IT systems offered and determined that the purchase complies with the restrictions on acquiring moderate or high-impact IT systems.
3) Responsibility Information

Offerors may provide any additional information, not already addressed under another proposal volume, that is considered to be necessary to demonstrate status as a responsible offeror under FAR 9.104, Standards.

L.26 MODEL CONTRACT AND ADDITIONAL REQUIREMENTS – VOLUME V
Instructions for Preparation of the Model Contract. Offerors shall submit with its proposal, in Volume V, the Model Contract in its entirety (basic and any amendments) per the instructions below.
