About this file
This document is a Summary of Benefits and Coverage (SBC) for the Manor Independent School District's BlueCross PPO health plan effective September 1, 2025, through August 31, 2026. The plan is offered through Blue Cross and Blue Shield of Texas as a PPO product with coverage available on both individual and family bases. The SBC outlines the specific cost-sharing structure for covered medical services, including deductibles, copayments, coinsurance rates, and out-of-pocket limits for both in-network and out-of-network providers. In-network deductibles are set at $1,400 for individuals and $4,000 for families, while out-of-network deductibles are $2,000 and $6,000 respectively. The plan's out-of-pocket limits are $7,000 individual/$14,000 family for in-network services and $23,700 individual/$47,400 family for out-of-network services. The document specifies coverage details across multiple service categories including primary and specialist office visits, preventive care, diagnostic testing, prescription drugs, emergency services, hospitalization, mental health services, maternity care, rehabilitation services, and other specialized care.
The plan employs a tiered cost-sharing approach with in-network primary care visits at $50 copayment, specialist visits at $70 copayment, and preventive care covered at no charge before the deductible is met. Prescription drug coverage includes a $200 individual deductible and tiered copayments ranging from $20 for generic drugs to $95 for non-preferred brand drugs at retail, with mail order options available at higher copayment levels. Emergency room facility charges carry a $500 copayment plus 20 percent coinsurance, with the copayment waived if the patient is admitted. The plan excludes coverage for services such as acupuncture, bariatric surgery, cosmetic surgery, adult dental care, infertility treatment, and non-emergency care outside the United States, while covering services such as chiropractic care (limited to 120 visits annually), hearing aids, and routine adult eye care. The document includes three coverage examples demonstrating potential out-of-pocket costs for maternity care, diabetes management, and emergency fracture treatment, with member responsibility ranging from $2,020 to $3,510 depending on the service scenario.

The Summary of Benefits and Coverage (SBC) document will help you choose a health plan. The SBC shows you how you and the plan would share the cost for covered health care services. NOTE: Information about the cost of this plan (called the premium) will be provided separately.
This is only a summary. For more information about your coverage, or to get a copy of the complete terms of coverage, call 1-800-521-2227 or at
For general definitions of common terms, such as allowed amount, balance billing, coinsurance, copayment, deductible, provider, or other underlined terms, see the Glossary. You can view the Glossary at www.healthcare.gov/sbc-glossary/ or call 1-855-756-4448 to request a copy.
Important Questions Answers Why This Matters:
What is the overall deductible?
In-Network: $1,400 Individual / $4,000 Family Out-of-Network: $2,000 Individual / $6,000 Family
Generally, you must pay all of the costs from providers up to the deductible amount before this plan begins to pay. If you have other family members on the plan, each family member must meet their own individual deductible until the total amount of deductible expenses paid by all family members meets the overall family deductible.
Are there services covered before you meet your deductible?
Yes. Services that charge a copayment, emergency room services, certain preventive care, and In-Network diagnostic tests are covered before you meet your deductible.
This plan covers some items and services even if you haven’t yet met the deductible amount. But a copayment or coinsurance may apply. For example, this plan covers certain preventive services without cost sharing and before you meet your deductible. See a list of covered preventive services at www.healthcare.gov/coverage/preventive-care-benefits/.
Are there other deductibles for specific services?
Yes. $200 Individual drug deductible. There are no other specific deductibles.
You must pay all of the costs for these services up to the specific deductible amount before this plan begins to pay for these services.
What is the out-of-pocket limit for this plan?
In-Network: $7,000 Individual / $14,000 Family Out-of-Network: $23,700 Individual / $47,400 Family
The out-of-pocket limit is the most you could pay in a year for covered services. If you have other family members in this plan, they have to meet their own out-of-pocket limits until the overall family out-of-pocket limit has been met.
What is not included in the out-of-pocket limit?
Premiums, balance-billing charges, and health care this plan doesn’t cover.
Even though you pay these expenses, they don’t count toward the out-of-pocket limit.
Will you pay less if you use a network provider?
Yes. See www.bcbstx.com or call 1-800-810-2583 for a list of network providers.
This plan uses a provider network. You will pay less if you use a provider in the plan’s network. You will pay the most if you use an out-of-network provider, and you might receive a bill from a provider for the difference between the provider’s charge and what your plan pays (balance billing).
Be aware, your network provider might use an out-of-network provider for some services (such as lab work). Check with your provider before you get services.
Do you need a referral to see a specialist? No. You can see the specialist you choose without a referral.
