About this file
This is an Exhibit J.11 Response Introduction document for the Texas Department of Criminal Justice's Photo and ID Card procurement (RFP 696-BF-26-O033). The exhibit establishes requirements for respondents to provide introductory information as part of their proposal submission. Respondents must submit a clear description of their organization, capabilities, goals, and purposes, along with designated points of contact for all departmental inquiries and a complete list of authorized negotiators with contact information. The document does not specify response deadlines, site visits, bidders' conferences, award dates, or contract terms within this particular exhibit, as it focuses solely on the submission of organizational introduction materials required for evaluation.
The exhibit requires respondents to provide insurance carrier information and statements of liability availability, though insurance is not mandatory for proposal submission. Respondents must detail all similar services provided to the Department or other governmental agencies, including references with contact information and contract numbers. Additionally, respondents must disclose any contracts cancelled or terminated within the preceding five years with explanations and contact information from the terminating entities, list all pending legal actions and final judgment outcomes, and provide proof of proper Texas business certification and absence of outstanding debt to the State of Texas. The Department reserves the right to contact references to evaluate past performance and contractor history.
