About this file
The document is an evaluation scoresheet for a Request for Statement of Qualifications (RFSQ) for Fund Raising Vendors issued by the Salt Lake City School District, with the solicitation number DH2628-AFV. The scoresheet is designed to evaluate potential vendors for fundraising activities, with a comprehensive scoring system that assesses both mandatory requirements and technical criteria. The evaluation process uses a 1-5 point scale, where 1 represents a failing score and 5 represents an excellent proposal that exceeds requirements. The technical evaluation covers four main criteria: Fundraiser Details (40%), Vendor Qualifications & Experience (30%), References (20%), and Complaints/Problem Resolution (10%), with a total possible score of 500 points.
The scoresheet requires vendors to meet minimum mandatory requirements, including submitting a completed Attachment C, providing a W9 form, obtaining a State of Utah DOC Fundraiser Permit, presenting a valid business license, and submitting vendor documents and proposed contracts. To be considered for selection, vendors must achieve a minimum technical point threshold of 75% (375 points out of 500). The evaluation document is marked as a draft and is not subject to GRAMA (Government Records Access and Management Act) pursuant to Utah Administrative Code. The scoresheet provides a structured framework for systematically assessing and comparing potential fundraising vendors for the Salt Lake City School District.
