About this file
This is a contractor compliance certification document for the City of Fort Worth related to Project No. 106821, a 2027 Asphalt Resurfacing Contract in Texas. The document requires contractors to certify compliance with Texas Labor Code Section 406.096 regarding worker's compensation insurance coverage for all employees working on the project. The major work scope includes approximately 2,710 linear feet of 7-inch curb and gutter, 15,221 square feet of 6-inch concrete driveway, 17,248 square feet of 4-inch concrete sidewalk, 50,465 square yards of 2-inch asphalt pavement resurfacing with surface milling, 15,635 square yards of 3-inch asphalt pavement with 11-inch pavement pulverization, 253 tons of cemlime, and 76 barrier-free ADA ramps.
The document requires the contractor to execute a certification statement and provide a notarized acknowledgment confirming worker's compensation insurance coverage. Additionally, contractors must submit certificates of compliance from all subcontractors demonstrating their worker's compensation coverage as required by Texas Labor Code Section 406.096(b). The certification must be completed with the contractor's printed company name, address, signature, and title, along with notarization by a Texas notary public before submission to the City of Fort Worth. This compliance certification is a standard construction specification document and a mandatory requirement for contract execution.
