A "customer" is any entity, except the Federal Government, which acquires supplies or services from the Offeror. The term customer includes, but is not limited to original equipment manufacturers, value added resellers, state and local governments, distributors, educational institutions (an elementary, junior high, or degree granting school which maintains a regular faculty and established curriculum and an organized body of students), dealers, national accounts, and end users. In any instance where the Offeror is asked to disclose information for a customer, the Offeror may disclose information by category of customer if the offeror's discount policies or practices are the same for all customers in the category. (Use a separate line for each customer or category of customer.)
Identify the type of Agreement. Indicate the type of agreement such as cost-per-test, cost-per-reportable-result, reagent rental, cost-per-billable etc. Offeror should indicate whether the agreement is sole source, dual source or multi source and whether there is a minimum commitment requirement.
Identify the effective date of the Agreement. Indicate the effective date of the agreement.
Identify the discount or discount range for reagents and consumables. The term "discount" is as defined in solicitation clause 552.212-70, Preparation of Offer (Multiple Award Schedule). Indicate the discount or discount range (based on your written discounting policies or standard commercial discounting practices if you do not have written discounting policies) utilized as part of the pricing methodology of the agreement for the customer or category of customer identified in column 1. Provide the commercial catalog list price, the awarded Federal Supply Schedule (FSS) Price under General Services Administration’s (GSA) FSC Group 66, Part II, Section N - Chemistry, Biochemistry, Clinical Instruments, General Purpose Laboratory Instruments and Services (if applicable) and your firm’s best commercial customer’s net price.
Average price of all reagent and supplies at the midpoint of pricing schedule. Provide a monthly figure that represents the cost of all reagents, calibrators, controls and supplies required to perform the assays offered at a given volume/or midpoint on the offered pricing matrix. The figures disclosed should be based on your firm’s best commercial customer’s net prices for all the items in question (discounted prices). Additionally, provide a statement of the basis used in calculating this figure.
Average price of service and maintenance for a one-year period. Provide the cost of service and maintenance on an annual basis, exclusive of the first year warranty period. Acceptable disclosures include extended service or warranty agreements offered to your firm’s best commercial customer and any extended service or warranty agreements awarded under FSS for FSC Group 66, Part II, Section N – Chemistry, Biochemistry, Clinical Instruments, General Purpose Laboratory Instruments and Services (if applicable). Additionally, provide a statement for the basis of this figure.
Average price to train one individual. Provide the commercial catalog list price, your firm’s best commercial price, and the awarded FSS price under FSC Group 66, Part II, Section N (if applicable) charged for a single training session of one or more individuals on the equipment offered. Also provide the number of individuals that are normally trained when the equipment is purchased. (The cost of which is included in the price of the equipment.)
Fair market value of analyzer after 5 years. Provide the residual value of the equipment after five (5) years. Provide a statement for the basis of this figure.
Complete this form by disclosing prices given to your commercial customers who receive your best discounts or net prices for each analyzer being offered. Such prices should be in effect on the date the offer is submitted or contain an effective date during the proposed multiple award schedule contract period. The Government expects you to provide information required by the format in accordance with these interactions that is, to the best of your knowledge and belief, current, accurate, and complete as of 14 calendar days prior to its submission. You must also disclose any changes in your price list(s), discounts or discounting policies, and/or net prices that occur after the offer is submitted, but before the close of negotiations. Attach. 3a WORLDWIDE DELIVERY Attachment 3a, FSS 66III Cost Per Test CPT/CPRR Pricing Model The proposed COST-PER-TEST (CPT) and COST-PER-REPORTABLE-RESULT (CPRR) prices (and/or options) shall be based on annual year (1-5) requirements predicated on consecutive years of contract CPT or CPRR participation subject to availability of funds. Incremental Analyzers may be provided at a lesser per-test price. Offeror should support lower prices through submission of matrices (in any form) with offer.

Pricing agreed to under a previous contract which has been granted to an ordering facility beyond the term of the respective contract may also be offered under this solicitation as a separate pricing structure. The extended pricing is restricted to those orders being placed to fulfill the terms of the agreement. All other terms and conditions of this contract will be applicable to the extended pricing. Varying matrices or pricing schedules shall be provided with offer to accommodate the aforementioned circumstances.

FOR COMMERCIAL SALES PRACTICES FORMAT
| In response to Attachment 2 - WorldWide Delivery Price proposal, number 2a (FSS 66 III Cost-Per-Test Commercial Sales Practice (CSP) Disclosures): (1) Provide the total amount of sales by your firm to the general public, including state and local governments, during the previous 12-month period or Offeror’s fiscal year. Be sure to identify the beginning and ending date of the 12-month period disclosed. (2) Show your total annual projected sales to the Government under this contract for each SIN offered under this contract. (3) Are the prices which you are offering the Government equal to or better than that offered to any customer? Indicate by selecting Yes or No. |
| In response to Attachment 2 - WorldWide Delivery Price proposal, number 2b, (FSS 66 III Cost-Per-Test Commercial Sales Practice (CSP) Spreadsheet), complete the chart for each commercial cost-per-test/cost-per-reportable result laboratory agreement based upon the volume of tests at the midpoint of the offered pricing matrix. Such agreement shall be in effect on the date the offer is submitted or contain an effective date during the proposed multiple award schedule contract period. In the event that your firm does not currently have a cost-per-test/cost-per-reportable result laboratory agreement, provide information on similar type of commercial agreements such as reagent rental, cost-per-billable etc. |
| The Government expects you to provide information required by the format in accordance with these instructions that is, to the best of your knowledge and belief, current, accurate, and complete as of 14 calendar days prior to its submission. You must also disclose any changes in your commercial agreements, commercial price list(s), discounts and/or discounting policies, which occur after the offer is submitted, but before the close of negotiations. If your commercial agreements vary by model or product line, the information should be by model or product line as appropriate. The contracting officer may require submission of these agreements. To expedite evaluation, offerors may provide a copy of these agreements at the time of submission. |
| Column 1--Identify the applicable customer or category of customer. A "customer" is any entity, except the Federal Government, which acquires supplies or services from the Offeror. The term customer includes, but is not limited to original equipment manufacturers, value added resellers, state and local governments, distributors, educational institutions (an elementary, junior high, or degree granting school which maintains a regular faculty and established curriculum and an organized body of students), dealers, national accounts, and end users. In any instance where the Offeror is asked to disclose information for a customer, the Offeror may disclose information by category of customer if the offeror's discount policies or practices are the same for all customers in the category. (Use a separate line for each customer or category of customer.) |
| Column 2--Identify the type of Agreement. Indicate the type of agreement such as cost-per-test, cost-per-reportable-result, reagent rental, cost-per-billable etc. Offeror should indicate whether the agreement is sole source, dual source or multi source and whether there is a minimum commitment requirement. |

| Column 4--Identify the discount or discount range for reagents and consumables. The term "discount" is as defined in solicitation clause 552.212-70, Preparation of Offer (Multiple Award Schedule). Indicate the discount or discount range (based on your written discounting policies or standard commercial discounting practices if you do not have written discounting policies) utilized as part of the pricing methodology of the agreement for the customer or category of customer identified in column 1. Also, identify the commercial price lists, which are the basis of the discounts given to the customers identified in the chart. The contracting officer may require submission of these price lists. To expedite evaluation, offerors may provide these price lists at the time of submission. |

| Complete this form by disclosing prices given to your commercial customers who receive your best discounts or net prices for each analyzer being offered. Such prices should be in effect on the date the offer is submitted or contain an effective date during the proposed multiple award schedule contract period. The Government expects you to provide information required by the format in accordance with these instructions that is, to the best of your knowledge and belief, current, accurate, and complete as of 14 calendar days prior to its submission. You must also disclose any changes in your price list(s), discounts or discounting policies, and/or net prices that occur after the offer is submitted, but before the close of negotiations. If your discount practices vary by model or product line, the discount information should be by model or product line as appropriate. Please provide a CPT/CPRR Price Analysis Disclosure Form for each analyzer offered. Use attachments if necessary. |

| Item 3 – Average price of service and maintenance for a one-year period. Provide the cost of service and maintenance on an annual basis, exclusive of the first year warranty period. Acceptable disclosures include extended service or warranty agreements offered to your firm’s best commercial customer and any extended service or warranty agreements awarded under FSS for FSC Group 66, Part II, Section N – Chemistry, Biochemistry, Clinical Instruments, General Purpose Laboratory Instruments and Services (if applicable). Provide the commercial catalog list price, your firm’s best commercial price, and the awarded FSS price under FSC Group 66, Part II, Section N (if applicable) charged for a single training session of one or more individuals on the equipment offered. Also provide the number of individuals that are normally trained when the equipment is purchased. (The cost of which is included in the price of the equipment.) |
