About this file
This document is a Draft Past Performance Questionnaire (RFP No. 77344426R0001) from the U.S. International Development Finance Corporation designed to collect detailed performance feedback about a contractor's previous work. The questionnaire is divided into two main sections: Section A to be completed by the offeror requesting a reference, and Section B to be completed by the individual providing the reference. Section B includes a comprehensive performance evaluation across six key elements: (1) Quality of Product or Service, (2) Schedule, (3) Cost Control, (4) Business Relations, (5) Management of Key Personnel, and (6) Utilization of Small Business. The evaluation uses a rating scale from Exceptional (E) to Unsatisfactory (U), allowing the reference to provide nuanced feedback on the contractor's performance. The final section includes open-ended questions about overall performance, strengths, weaknesses, and the potential for future contract awards.
