About this file
This is a Pre-Proposal Request for Information (RFI) log documenting 60 clarification inquiries submitted by prospective bidders for the Whiteriver Hospital Construction project (Solicitation 75H70126R00016) during the pre-bid period. The RFIs address design ambiguities and specification conflicts across civil, architectural, interior, electrical, ICT, and landscape disciplines for a 407,000 square-foot hospital construction on the Fort Apache Indian Reservation in Whiteriver, Arizona.
Key clarifications provided include: headwall procurement methodology (Contractor Furnished/Contractor Installed confirmed); civil infrastructure details for lift stations, storm drain materials (accepting CMP Spiral rib and RGRCP as alternates to 72" HDPE), catch basin and manhole specifications, and cattleguard sizing; architectural elements including door types (SL doors are barn-door style sliding doors; hospital stops required only in heavily sanitized areas), exterior panel materials (ACM confirmed over metal panels), and handrail specifications (HR-1 at WP-01 locations, HR-2 at WP-03 locations); interior finishes such as melamine color (white specified), QTZ material selection (QTZ-1 confirmed at Public Waiting areas, QTZ-2 as substitute for QTZ-3), and cabinet lock requirements; electrical specifications clarifying unshielded Category 6A cabling with matching connectors; native seeding methodology (70% grass mix/30% wildflower mix; hydroseeding and drill seeding acceptable; straw mulch required at 2 tons per acre); and medical equipment procurement (owner-furnished versus contractor-furnished designations). The Owner anticipates a three-month move duration from the existing to new hospital following Phase 1 final acceptance. CAD drawings and BIM models are not available during the bid period but may be discussed post-award. Design Team responses indicate multiple addenda forthcoming to address identified discrepancies and provide updated drawings.

1 4/1/26 CSI 10 or 11 I was unable to locate headwall specifications within the current document set (usually in CSI 10 or 11). To allow us to prepare accurate pricing, we are requesting clarification on the following items:
Before reading the bullet point list, please confirm that you will take a quote for premanufactured Headwalls and will not be building as you go (for instance the single gas outlet in H-19C I can see as being built as you go, but H-21 Inpatient Room looks more like a premanufactured headwall). If this is correct, please answer the following:
| AE | The Design Team is open to Premanufactured Headwalls; the current Documents represent a stick-built method. If GC would like to pursue this method then a Substitution Request should be provided for review / approval. | |||||
| 1. Procurement Designation – Will the headwalls be Owner Furnished/Owner Installed (OFOI), Owner Furnished/Contractor Installed (OFCI), or Contractor Furnished/Contractor Installed (CFCI)? (do you have a list of bidding GC’s if CFCI?) | AE/COR | The headwalls are CFCI, but we do not maintain a list of bidding GC's. | ||||
| 2. Panel Supply – Would you like the headwall supplier to furnish the wall panels as part of their scope, or will panels be provided separately? If so, should we supply the specialty sheet wall protection when called out? | AE/COR | This is up to the GC. IHS expects the GC to provide the entire wall assembly including wall panels and headwalls. FYI…there are custom patterns the Headwalls in some areas that will need to be coordinated with the Finish Schedule. | ||||
| 3. Accessories – The plans show accessories attached to the headwalls. Should these accessories be included in the headwall supplier's scope of supply? | AE | The Design Team is open to Premanufactured Headwalls; the current Documents represent a stick-built method. If GC would like to pursue this method then a Substitution Request should be provided for review / approval. | ||||
| 4. Pricing Format – Would you prefer pricing submitted as a complete unit (headwall with all accessories) or broken out as individual accessory line items? Could you please confirm the above at your earliest convenience? Once we have that clarification, we will prepare and submit pricing to the appropriate party. | AE/COR | This is up to the GC. Pricing Format provided by the GC to IHS must follow the format outlined in the solicitation, but the pricing format between the GC and supplier is determined by the GC. | ||||
| 2 | 4/13/26 | BP3_05_Civil-IHS-Hon-Dah-UC_Final-CD plan sheet C8.03 the details are on C9.03 | Is the lift station new or existing? | |||
| a. | The detail shows up on 3 different plan sets received. | |||||
| b. | There’s a note on one that says CORE EXISTING SEWER LIFT STATION and grout around reclaimed line … | |||||
| c. | There’s no depth for the diversion MH on the detail – is this new or existing? | AE | In the BP3 package, there are two lift stations, one existing at the north end of the project, and a new lift station adjacent to the new WWTP plant building. | |||
| A. | Details for the new lift station would be part of the BP3 set and are shown on page C9,03 | |||||
| B. | Correct, this line is to allow the WWTP to potentially drain its contents back in to the tribal sewer system. The only way to get the wastewater in the existing lift station would be to core the existing lift station for the new pipe penetration. | |||||
| C. | The Diversion MH is shown on page C5.05 in profile view with the inverts of the pipes. |
| 3 | 4/13/26 | BP1_05_CIVIL_IHS-Whiteriver-HOSPITAL-Plans_Final-CD Plan Sheet CD C3.05 | What size cattle guard is needed? | AE | Refer to sheets C3.05, C3.06, and C3.07 for cattleguard information and size (overall width of street and curb and gutter) is the length of cattleguard. Also refer to ADOT STD DET C-11.10. If further information is needed, please submit another RFI and provide references. | ||
| 4 | 4/13/26 | C4.05 | On sheet C4.05 the plans are calling for 72" HDPE storm drain when 60" is the largest size offered. Can we provide an alternate of CMP Spiral rib? | AE | We would accept alternate materials that meet the pipe sizing noted, if using CMP Spiral rib, or other alternative material, smooth interior walls are required, if RGRCP then Class II would be required along with the smooth interior walls. | ||
| 5 | 4/13/26 | C3.03 & C3.04 | Is there more detail on the storm drain catch basins off of the water tanks calling for a Sump inlet pages C3.03 & C3.04? | AE | Reference notes have been clarified in drawings provided in addendum A | ||
| 6 | 4/13/26 | C4.11B | Going through the drainage on phase two I found a discrepancy on C4.11B (page 111). On the top plan it shows 24" for a run and then on the profile it shows 18" | AE | 24" is the correct pipe size | ||
| 7 | 4/13/26 | As I get into the plans for the hospital I'm noticing pipe footage differences between the plans and the profiles. It's only on random sections | AE | 4/14 - Plan and profiles will be checked and updates issued in a future addenda as required. 4/29 UPDATE: After Further review, there are no locations where pipe lengths shown in plan view are also called in a profile. | |||
| 8 | 4/13/26 | The catch basins and manholes have the same SD with a circle around it, do we follow the call out and numbers to figure out what they are? | AE | Yes, please plan to build per the construction note call outs | |||
| 9 | 4/13/26 | 10-2600 | Both line type WP-01 and WP-03 call out handrail but the finish legend shows 2 different types (legend below). Where does each go? HR-1 CS ACROVYN HRBW-10CHL WOOD GRIP, EGGSHELL ACROVYN | ||||
| HR-2 CS ACROVYN HRWS-6C WOOD GRIP See Image to the right. | AE | At WP-01 locations se HR-1. At WP-03 locations use HR-2. | ERROR:#VALUE! | ||||
| 10 | 4/13/26 | Various | Has an exterior mockup been designed? Should an allowance be established for this? | AE | No Exterior Mockup(s) have been created in Design Phase but there should be an allowance / budget dollars set aside for this. The Goal is to create "Max Value Mockups;" with all the Stakeholders involved (Design / Owner / GC / Trades) Mockup(s) development will be the result of preconstruction meetings / activities. | ||
| 11 | 4/13/26 | AF Series | The hardscape sheets and roof plan reference AF Series for the type and quantities for the Courtyard and Roof tables and chairs. Can you please provide the referenced AF series information for type and quantity? | AE | Note on AF series confirm all tables and chairs are part of a FFE contract. Outdoor waste cans and bike racks to be included with in the hardscape plans and contractor bid | ||
| 12 | 4/13/26 | LS2.0.0 | The Anti-Ram Helio Bollards with Security Core show both Illuminated and Non-Illuminated models in detail 1 – L5.02. | 13 | 4/13/26 | Finish Schedule | Please provide specification for QTZ-3 | AE | Use QTZ-2 in leu of QTZ-3. | ||
| 14 | 4/13/26 | 27-0000 | Division 27 (217500-4) states Category 6A cable, and the part number provided is for unshielded cable (Berktek 11101842). Connectors are noted as shielded jacks (271550-5, 2.4, F2.a). Patch cords are also noted as shielded (271500-6, 2.4, I5.c). Please clarify that connectors and patch cords need to be shielded or can be adjusted to match unshielded Cat 6A cable. | AE | Connectors shall be adjusted to match unshielded Category 6A cabling. Patch cords currently specified are suitable for unshielded applications. References to shielded cabling shall be removed as required. | ||
| 15 | 4/13/26 | 08-1113 | Per the project manual, section 081113 Part 2.7.B.4, it states that “Terminated Stops (Hospital Stops): Terminate stops 6 inches above finish floor with a 45 -degree angle cut, and close open end of stop with steel sheet closure. Cover opening in extension of frame with welded-steel filler plate, with welds ground smooth and flush with frame.” Does this mean that every door frame fabricated on this project needs to have hospital stops? | AE | Heavily Sanitized Areas such as Procedure, Surgery, etc. would require the "Hospital Stop" Door Frames…Rooms such as Offices would not require these. | ||
| 16 | 4/13/26 | 08-1473 | Please provide clarification for elevation ‘SL’. There is conflicting information about what is wanted. Do the openings that have elevation ‘SL’ need to follow what the hardware set specifies, or do they need to follow specification section 081473? | AE | To clarify SL-Doors are "barn-door" style sliding doors. To further clarify the Door Elevation have been updated to show both SCW Doors and GL Door Types. Please follow the Spec Section 087100 regarding these doors. (Will include in forthcoming Addendum) | ||
| 17 | 4/13/26 | 08-1473 | a. If the answer to the above question is to follow specification section 081473, then will it be acceptable to provide wood doors from two different manufacturers? Specification section 081416 Part 2.1.A states that wood doors must be sourced from a single manufacturer | AE | Please follow the Spec Section 087100 regarding these doors. Single-Sourced is required for consistency. | ||
| 18 | 4/13/26 | Door Schedule | 1. Please provide clarification for elevation ‘SL’ but with GL as the door material. There is conflicting information about what is wanted. Are these openings full glass doors or are they wood doors with a glass insert in them? | AE | To clarify SL-Doors are "barn-door" style sliding doors. To further clarify the Door Elevation have been updated to show both SCW Doors and GL Door Types. (Will include in forthcoming Addendum) | ||
| 19 | 4/13/26 | Door Schedule | a. If the answer to this question is full glass, please be aware that hardware set 178.0 will not work with a glass door. | AE | To clarify this will be a Aluminum style-framed full glass door (not frameless)- refer to Door Elevations A9.3.7. (Will include in forthcoming Addendum) Therefore Hardware Set 178.0 will work. | ||
| 20 | 4/13/26 | Door Schedule | Please provide a door and hardware specification for opening # IMG-1235A-2. Serenity door sliding system does not have an option to provide a lead lined slider door. | AE | Its not anticipated that this Door will require "Lead-Lined" but will need to verify once a site-specific "Physicist Report" to verify if this door is affected. | ||
| 21 | 4/13/26 | A4.1.1 | The South elevation on page A4.1.1 calls out the top band of panels at 82' high between grid 5 and grid 27 as both A409 and SW-2. A409 is defined as "12" ACM, CUSTOM COLOR TO MATCH TERRA COTTA" while SW-2 is defined as "12" WIDE FLUSH METAL PANEL WITH 1" REVEAL". This band runs the circumference of the building. Is it intended to be ACM or a single skin flush metal panel? | AE | ACM Panel to match the Terra Cotta; refer To Detail 20 / A5.5.5 for the Rooftop Mechanical Screen; (Additional Details 22, 23, 24 / A5.5.5) | ||
| 22 | 4/13/26 | AI3.1.1C | The image to the right is a portion of the 1st floor drawing AI3.1.1C. The highlighted note AI305 says all walls to have WP-04. WP-04 is .040 4’ high. The location of the note is not near a room name so it is unclear where it is referring to. There are several rooms and corridors that are open to this area (bigger than the portion below) so I am unsure where to start and stop the sheet. Can you get clarification to the extent of the WP-04 in this area? | AE | Pharmacy wall protection drawings forthcoming in Addendum B. | ERROR:#VALUE! | |
| 23 | 4/13/26 | AI3.1.1D | In the picture to the right, WP-05 is full height SP-1 (.040 sheet), WP-08 is full height decorative sheet wall protection and says SP refer to elevations. There are SP-1 through 8 and there are no elevations. Can you get clarification on WP-08? | AE | Use SP-6 at both Patient Prep MRI (Zone II) IMG-1226A & IMG-1226B at WP-08 called out locations. | ERROR:#VALUE! | |
| 24 | 4/13/26 | Our drafting team is working on a material list for this project for our bid proposal. We are unable to identify the referenced rooms with patient lifts. Please advise if these are different room numbers or if these rooms still need lifts. | AE | WEL-1114C, WEL-1114D, and IPC-3199 do not have patient lifts. | 33 | 4/20/26 | Can the BIM model be made available for bidders to reference? | COR | No, we recognize that this would assist in performing take offs, but the files are too large and the risk to the designer is too great for the government to share these documents with the public. AE Concurs; after Bid Period access to BIM Models and Backgrounds will be discussed with GC at a later date once all E-Media Disclosure Forms have been signed. | |
| 34 | 4/20/26 | BP1 Civil Hospital, Starting at C4.13 | The print shows a Manholes but calls out a catch basin. How do I bid it? | AE | Please bid per the construction notes on the plans. |
| 35 | 4/20/26 | BP1 Civil Hospital, C4.14 | The curb inlet CB is 36” ID, the pipe is 36” but the CB is at such a angle there is not enough room for the pipe. |
We can make it work by pouring a bottom first, then setting a riser on top. Is that how I should bid it?
| AE | There are no catch basin sizes called out in the plans. Confirm based on manufacturer specifications, or MAG details what size inlets are needed, based on the referenced construction note. Means and methods of installation cannot be answered by the design team. | |||||
| 36 | 4/20/26 | BP1 Civil Hospital, C4.16 | Do I bid all the catch basins as called out being a 2’ X 3’ or are the smaller ones going to be something else? | AE | Please bid per the construction notes on the plans. | |
| 37 | 4/20/26 | BP1 Civil Hospital, C4.17 | The MAG spec is for a 48” MH but the pipe is 48”. Do I just increase the MH to a 60”? That would be for all the 42” pipe also. | |||
| Also, all the MHs are calling for a grated lid. Is that what should be bid. They cost more. | AE | The referenced MAG detail is for pipes 48" and smaller, as this is a 48" pipe, the MAG 520detail would apply. Please bid what is called out in the construction notes | ||||
| 38 | 4/20/26 | BP1 Civil Hospital, C4.18 | The CBs are a MAG 532 with a sidewalk top, could we do a double MAG 534 curb inlet? | AE | Please bid per the construction notes on the plans. | |
| 39 | 4/20/26 | BP1 Civil Hospital, C4.21 | Drawing show a MAG 535 double but a single is called for. | AE | The inlet at station 0+83 is intended to be a double 535 inlet | |
| 40 | 4/20/26 | BP1 Civil Hospital, C4.22 | Again small pipe with 2X3 CB. | AE | Please bid per the construction notes on the plans. | |
| 41 | 4/20/26 | BP1 Civil Hospital, C4.08 | Shows double CBs but calls for singles | AE | Please provide a double curb inlet. | |
| 42 | 4/23/26 | I have been combing through the plans online and am unable to find the wayfinding sign package. Can you please help point me in the right direction for this? | COR | The wayfinding drawing set is called “BP1_18_WAYFINDING_IHS-Whiteriver-Hospital_FINAL-CD”. AE Concurs | ||
| 43 | 4/23/26 | Are the manufacturers listed in the specs the only manufacturers allowed to bid that particular equipment or will other manufacturers be allowed as long as it meets schedule and specifications? | COR | 1. In general, yes, other manufacturers are allowed to bid the equipment. The manufacturers just have to meet the requirements of the specs. Look for language such as “or equal” or “but are not limited to” in the manufacturer section. For clarity, the manufacturers listed meet the intent of the design. Any other manufacturer that meets the specifications and are similar in form and function to those listed are acceptable. |
2. There are some exceptions to this, which do not include language such as “or equal” or “but are not limited to” in the manufacturer section. For example (not an exhaustive list):
a. The IT equipment is specifically required by the Area IT department. These are things like the wireless access points, servers, switches, etc. which have specific functionality and compatibility which they need to be operationally efficient.
b. Some medical equipment has to meet specific requirements, for example:
I. The medical gas port connections are specific to the systems they are currently using. This is necessary for compatibility with the medical gas system they use for all their medical gas connected equipment. Each type of gas connect has a specific shape that makes in virtually impossible to connect a given device to the wrong port. This is a patient safety issue.
ii. Some equipment has to communicate via a specific protocol to it can function with other IHS systems.
c. Camera systems and access control systems are specified for compatibility with other systems used across the Area. This is specifically necessary for the Pharmacy CCTV system.
d. The Building Automated System (BAS), must be Automated Logic. The Phoenix Area uses this system exclusively because there is an area-wide server which communicates with all controllers and staff are able to program and control equipment through this area wide system. If another manufacturer was used, this functionality would be degraded.
44 4/23/26 Medical Equipment I'm working with multiple manufacturers responding to the Whiteriver project RFPs for medical equipment. I haven't been able to locate or get a response regarding Bill To and Ship To addresses for equipment, such as medical supply carts. Can you please provide that information? COR Owner furnished medical equipment:
1. Owner furnished medical equipment is not delivered or billed under this contract.
2. Bill To and Ship To addresses for, owner furnished medical equipment will be included in the solicitation for these contracts.
3. I think the medical supply carts are owner furnished equipment Contractor furnished medical equipment:
1. Delivery of contractor furnished medical equipment, which is included in this contract, will be arranged by and with the GC. The GC may want to deliver it to the hospital site (200 W Hospital Way, Whiteriver, AZ), but that needs to be done in accordance with section 016000 of the specs, or the GC might want to deliver it to a warehouse for storage until such time that they are delivered to the site. That will be entirely up to the GC.
2. Billing for the contractor furnished medical equipment will be to the GC or their sub as they arrange.
45 4/23/26 Do equals need to be approved before bid submission? COR 1. Per H.17 of the solicitation, “Contractor proposed equivalents shall be submitted for review after award as a Request for Information (RFI). Proposed equivalents will not be reviewed during the solicitation period.”
2. Per Spec Section 016000-2.1.A.6 “Or Equal: For products specified by name and accompanied by the term "or equal," "or approved equal," or "or approved," or "or approved comparable product," comply with requirements in "Comparable Products" Article to obtain approval for use of an unnamed product.
a. Submit additional documentation required by Architect in order to establish equivalency of proposed products. Unless otherwise indicated, evaluation of "or equal" product status is by the Architect, whose determination is final.” AE Concurs
| 46 | 4/23/26 | BP1 Civil Hospital, C4.05B | In Civil Plans, C4.05B is missing from the Phase 2 Grading & Drainage Plan. It appears C4.05 from the Phase 1 Grading & Drainage Plan was used instead. Please advise. | AE | Sheet C4.05B will be provided as part of Addendum B |
| 47 | 4/23/26 | BP1 Arch A1.2.1 | Drawing A1.2.1 (BP-1 Hospital) shows three rolling gates under Keynote 120. Kindly confirm their height. Also, Drawing A1.3.2 provides details, but framing size is not indicated—please advise. | AE | The Rolling Gates are desired to Match the Height of the T.O.Walls - refer to Enlarged Elevations on Sheets 72/A4.4.4 & 72/A4.4.10...confirm with Vendor top track mounting height if required as noted on Detail 53B/A 1.3.2. | 49 | 4/23/26 | A5.6.1 | Please see comments for 042500 C.04 and C.05 | AE | Design Team is open to Bidder's suggestion regarding "Black" Trim pieces to maintaining the desired appearance and strength of material; if GC would like to pursue this method then a Substitution Request should be provided for review / approval. |
| 50 | 4/23/26 | A4.4.1 | Please clarify between ACM, Trespa and K-wall | AE | ACM is the desired Material for the Exterior Wall Panels and Soffits shown in these Exhibits. Will clarify in forthcoming Addendum. |
| 51 | 5/4/26 | 32-9220 | Native Seeding – Seed Mix / Basis of Bid: The landscape plans identify a Pinon Juniper Meadow Mix for native seeding, while Specification Section 329220 references separate Native Grass Seed Mix and Native Forb Seed Mix installations, including phased scheduling requirements under Part 1.7.A.3, 1.7.A.4, and 1.7.A.5. | AE | Seed mix (species composition and PLS) to be fully updated as part of upcoming Addendum C. Use a ratio of 70% Pinon Juniper Grass mix by Western Native Seed (2lb/1000SF) and 30% Pinon Juniper Wildflower mix by Western Native Seed (8oz/1000SF) as temporary basis of design. Spec will be updated as part of upcoming Addendum C to remove any reference to Native Forb Seed Mix. |
| Please confirm the required seed mix basis for bidding and indicate which document governs. | AE | Use seed mix information found on LP100 series drawings. | |||
| a. If separate Native Grass and Native Forb seed mixes are required, please provide species composition, PLS rates, and application rates for each mix. | AE | N/A | |||
| b. If Pinon Juniper Meadow Mix is required, please provide species composition, PLS rates, and application rate for that mix, and clarify whether the two-step installation and maintenance requirements in Section 329220 apply to that single mix. | AE | Two-step installation and maintenance requirements in section 1.7 do not apply and will be removed from spec. Follow maintenance guidelines per section 1.9 and installation guidelines per section 3.3 of updated spec in upcoming Addendum C. | |||
| 52 | 5/4/26 | 32-9220 | Native Seeding – Installation Method: Specification Section 329220, Part 3.1.D identifies hand-broadcasting and drill/drop seeding methods, but does not reference hydroseeding. Please confirm whether hydroseeding is an acceptable installation method for native seed areas. | AE | Hydroseeding is an acceptable installation method for native seed areas where site conditions are not appropriate for drill seeding, and will be added to installation methods section in upcoming Addendum C. Drill seeding is the preferred method. |

| Civil | Complete | Complete | 4/3/26 | 4/2/26 | ||||||
| 1. Procurement Designation – Will the headwalls be Owner Furnished/Owner Installed (OFOI), Owner Furnished/Contractor Installed (OFCI), or Contractor Furnished/Contractor Installed (CFCI)? (do you have a list of bidding GC’s if CFCI?) | AE/COR | The headwallsare CFCI, but we do not maintain a list of bidding GC's. | Civil | Complete | Complete | |||||||
| Civil | Complete | Complete | |||||||
| Civil | Complete | Complete | |||||||
| Civil | Complete | Complete | |||||||

| a. | The detail shows up on 3 different plan sets received. | |||||||||||
| b. | There’s a note on one that says CORE EXISTING SEWER LIFT STATION and grout around reclaimed line … | |||||||||||
| c. | There’s no depth for the diversion MH on the detail – is this new or existing? | AE | In the BP3 package, there are two lift stations, one existing at the north end of the project, and a new lift station adjacent to the new WWTP plant building. | |||||||||
| A. | Details for the new lift station would be part of the BP3 set and are shown on page C9,03 | |||||||||||
| |||||||||||
| |||||||||||
