About this file
This is a Lead-Based Paint (LBP) Sampling Report for the Gallup Indian Medical Center (GIMC) campus in Gallup, New Mexico, prepared by iina ba, Inc. under IHS Contract No. HHSI161201100001C. The inspection was conducted between January 11-17, 2012, using X-ray Fluorescence (XRF) analysis to identify lead in paint on interior and exterior surfaces across 13 buildings. Lead-based paint was detected at or above the HUD threshold of 1.0 mg/cm² on multiple building components, with positive findings in Buildings 2000, 2001, 2002, 2003, 2005, 2007, 2009, 2017, 3000, 3001, and 4010. Affected components include ceramic wall tiles, exterior walls and trim, windows, doors, railings, fencing, and various metal components. The report identifies approximately 7,200 square feet of components with lead-based paint requiring remediation.
The report provides detailed cost estimates for lead abatement: chemical removal at $28,949.28 and component demolition at $17,776.77, both including Environmental Professional costs but excluding replacement materials. The document includes comprehensive XRF field data tables, photographic documentation, calibration certifications, and recommendations for safe handling procedures. The report emphasizes that LBP poses no health hazard when intact but recommends an Operations and Maintenance (O&M) program for periodic inspections and immediate cleanup of damaged surfaces. Major renovation or abatement work must be performed by licensed contractors in compliance with OSHA Lead in Construction standards. The file also contains hyperlinked data tables and functional space maps for each building inspected, providing detailed room-by-room sampling results.

1.0 EXECUTIVE SUMMARY
As requested by the Navajo Area Indian Health Service (IHS), a Lead-Based Paint (LBP) inspection was conducted on the Gallup Indian Medical Center Campus (GIMC) within the Gallup Service Unit located in Gallup, McKinley County, New Mexico. Sampling occurred between January 11, 2012 and January 17, 2012. The purpose of the inspection was to identify lead in paint on interior and exterior painted surfaces of each building. Measurements were made using a Niton X-ray Fluorescence (XRF) Spectrum Analyzer (Model XLp 303A). Specifications on this instrument are included in Appendix
4. Confirmatory chip sampling for laboratory analysis proved to be unnecessary due to the lack of indecisive XRF readings.
Iiná, bá, Inc (iiná bá) representatives Mrs. Judy Moore, Lead Inspector, and Mr. Nate Chase performed the asbestos inspections. The inspector holds current a United States Environmental Protection Agency (USEPA) accredited 24-Hour Inspector Certification.
A copy of the Certification can be found in Appendix 4.
Sampling procedures similar to the United States Department of Housing and Urban Development (HUD) Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in Housing (June 1995), and revised in 1997, were followed for each of the buildings. Within each building or area sampled, procedures similar to the HUD multi-family housing inspection, as outlined in Chapter 7 of the HUD Guidelines, were employed.
2.0 DESCRIPTION OF BUILDINGS INSPECTED
Building 2000 is a 136,726 square foot (sq. ft.), five-story Hospital building with ground level mechanical, computer support and storage rooms. The building resides on a small hill with the ground floor visible to the north and the southeast side butted against the hillside to the second floor. Mechanical units are located at the exterior southwest ground floor, southeast second floor and on the roof. The interior ceiling construction includes ventilation and plumbing runs between the ceiling and the preformed cement flooring above, hanging and attached ceiling tiles, a plaster ceiling system, and a drywall ceiling system. Interior wall construction includes drywall and spray on plaster, often covered by ceramic tiles. Interior flooring consists of various sheet vinyl flooring and floor tile, with ceramic tile flooring in bathroom, kitchen, and dining areas; and carpet and carpet tiles in waiting areas.
The ground floor computer room has a raised floor with metal tile flooring and a separate HVAC unit.
At the Contract Office Technical Representative’s (COTR) request, the second floor Operating Room area (rooms 283 to 300) was sampled using paint chip methods on August 10, 2011 by Ron Rhoades of Rhoades Environmental. This report and the “Bulk Sampling Form” are included with the iiná bá sampling forms and sampling tables.
During the sampling procedure, Room 297 was found to have severely damaged walls with evidence of metal plating in the walls. Further discussion with hospital staff revealed the room had been used as a radiology area.
Ceramic floor and wall tiles throughout the building were found to be lead containing.
Exterior walls consist of metal panels, glass and rock over cement building material
(CBM).
The first floor includes an annex on the east used as the eye clinic and another annex on the north used for the microbiology clinic. Both annexes are modular with wood frames on CBM support. The north annex exterior is wood and the east annex exterior is metal and rock. The interior are similar to the main building. The annexes also share some mechanical units, but have their own roof cooling units.
Three separate buildings are attached to, and accessible from, Building 2000. They are Building 3000, Medical Records; Building 3001, the main Pharmacy; and Building 3002, Housekeeping.
Building 2001 is listed as the Records building, but is in fact the 1,367 sq. ft. carpentry shop. The building is CBM on a concrete slab with interior wood frame. There are two garage doors closed in with wood. The roof is wood with tar shingle and a metal awning connecting the building to Building 3000.
Buildings 2002 and 2003 are small clinics with exam rooms, administration offices, and waiting rooms. Building 2002 is 1,575 sq. ft. and Building 2003 is 3,150 sq. ft. The interior walls are drywall systems with ceramic tile in the restrooms. The interior ceilings are drywall systems with plaster or hanging 2 ft. by 4 ft. wood ceiling tiles. The interior floor consists of vinyl and ceramic tiles. The exterior walls are wood and plaster systems over CBM and set on a cement slab. The eaves are plaster and wire mesh systems that are 1 to 3 feet wide around the buildings. The roofs are wood with tar shingle and HVAC systems on top.
Building 2005 was at one time separated from Building 2006 by a connecting wall.
The east side of the connecting wall has been removed forming an interior connecting hallway, and the building renumbered as Building 2005 with a total of 3,670 sq. ft. The building contains administrative offices. The interior walls and ceiling are drywall systems with ceramic tile in the restrooms. Interior flooring consists of various vinyl floor tiles. The ceiling consists of drywall systems and hanging ceiling tiles. The exterior is CBM covered by plaster and wood on the north and south sides. The eaves are plaster and wire mesh systems that are 1 to 3 feet wide around the buildings. The roofs are wood with tar shingle and HVAC systems on top.
Two CBM storage units with exterior dividing walls are attached to the building. The units’ interiors are unfinished wood frame.
Buildings 2007 and 2009 are administrative offices of 3,150 sq. ft. each. They are constructed similar to Building 2005 with each building connected to a single storage unit.
Building 2016 is 5,682 sq. ft., a two-story building containing nursing offices. The interior flooring consists of vinyl floor tile over cement and cement stairs. The interior walls along the exterior are a plaster wall system. The individual room walls are a drywall system, with ceramic tiles over the drywall system in the restrooms. The ceiling system consists of 12 inch by 12 inch square attached wood layer tiles over drywall. The mechanical room is on a split lower level with access tunnels under the building. The roof is preformed cement covered with a rubber roof system.
Many offices in this building have vinyl wall paper and wood paneling over drywall.
Building 2017 is a 5,758 sq. ft. modular building on CBM supports. The wood framed exterior is covered with plaster and cracks throughout the exterior walls indicate the building has been damaged from settling. The interior walls are drywall systems and the ceilings are 2 ft. by 4 ft. hanging tile systems. The roof is a rubber sheeting system.
Building 2018 is a three-story, 40,888 sq. ft., CBM and preformed cement building. The building was built less than 10 years ago, and at the request of the COTR was not sampled for lead-based paints.
Building 3000 is 8,000 sq. ft., metal framed building on a cement slab. The exterior is painted metal corrugated siding and roof. The interior large records storage area is open to the roof and exterior walls with fiber glass insulation. The office areas are drywall system walls and 2 ft. by 4 ft. hanging tiles on the ceilings. The floors are 12 inch by 12 inch vinyl tiles, 18 inch by 18 inch carpet tiles, and cement. The building is connected to the hospital emergency room by a metal roof walkway.
Buildings 3001 and 3002 are connected to the hospital at the main entrance and at the west side respectively. Building 3001 is 2,639 sq. ft. and 3002 is 4,400 sq. ft. They are metal frame buildings with exterior painted corrugated metal siding and roofing, set on cement foundations with CBM supports. The interior walls are drywall systems and the ceilings are drywall with hanging ceiling tiles and plaster systems. The floors are vinyl sheeting and vinyl tile on cement. Where Building 3001 connects with the main hospital entrance, the ceiling is 8 inch by 8 inch wood tiles attached to drywall.
Building 4008 has been demolished. At the request of Doug Clark, Building 4010 was inspected as a substitute. Building 4010 is approximate 3,500 sq. ft. modular wood frame building placed on CBM supports. The exterior walls are painted wood panel and the roof is tar sheeting. The interior walls are a drywall system with many walls on the exterior sides covered with wood paneling. The ceiling is a hanging 2 ft. by 4 ft. ceiling tile system. The flooring is vinyl 12 inch by 12 inch vinyl tiles and 18 inch by 18 inch carpet tiles over a wood subfloor.
Suspect building materials identified and sampled from the Gallup Indian Hospital Campus buildings included the following: ceramic wall tiles, exterior and interior wall paints and wood stains, vinyl wallpaper; and coated doors, windows and their trims.
3.0 SURVEY METHODS
3.1 XRF Instrumentation
Measurements of lead in paint were made using a Model XLp 303A ThermoFisher Scientific Niton XRF Spectrum Analyzer (serial number 26670). The XRF Lead Paint Analyzer non-destructively measures lead concentrations of painted surfaces. The XRF system includes three (3) main components: (1) the XRF analyzer, which is a portable analytical instrument operating on the principles of X-ray fluorescence; (2) forms for recording of field data, and (3) report generation software, which allows for generation of data tables.
The criteria for classifying results obtained using the XRF are as shown in Table 1 below. These results are taken from the HUD/EPA Performance Characteristics Sheet, specific for the XRF system used in this survey.
Table 1: LBP Classification Criteria, K-Shell Value

iiná, bá, (as a firm) and its Lead Inspectors are licensed by the State of New Mexico Environmental Department, Radiation Control Bureau as well as certified by the USEPA to perform LBP inspections. Copies of the certifications are included in Appendix 4.
3.2 XRF Calibration Check
Standard quality control procedures with respect to the XRF were performed during the survey. The internal calibration of the XRF was checked by taking a minimum of three
(3) consecutive measurements on a National Institute for Standards and Technology (NIST) standard with a known concentration of lead. The results of the calibration checks were then used to verify that the instrument was operating within acceptable limits. Periodic calibration checks were performed during the course of a day, specifically at the end of the day to “bracket” the test data collected between calibrations.
These calibration checks are reported within the XRF data tables found in Appendix 2 of this report and are maintained in a file to detect changes in instrument performance over time (sign of source deterioration).
3.3 Substrate Correction
For some XRF instruments, it is necessary to make corrections for bias in XRF readings made on some painted surface or “substrates”. In accordance with the HUD/EPA Performance Characteristics Sheet for the XRF analyzer used, it is not necessary to correct for substrate bias.
3.4 Field Documentation
Field data dorms were used to manually record information about individual XRF measurements. This information included the room number, the building component (such as wall, ceiling, door, baseboard, cabinet, etc.), component substrate (e.g., drywall, wood, metal, concrete), and paint condition. The field data forms were then used by the inspector to couple measurement-specific descriptive information to the sequential data once the instrument’s electronic memory was downloaded to a personal computer. This information was then manipulated by the instrument’s software to generate data tables for the written report.
According to the HUD Guidelines, each enclosed space in a structure (e.g., room, stairway, hallway, etc.) is considered a separate "room equivalent" and must be tested separately. To keep track of where individual measurement locations were made, interior and exterior walls of a structure were designated as A, B, C, and D, as shown in the attached drawings. The A wall was designated as the North wall in this inspection, and the B, C, and D walls are in clockwise order from the A wall. The wall and position on the wall (i.e., left, right, or center) was also recorded when readings were taken on components such as windows or doors. Any comments regarding a particular room were also recorded and may be found in the data tables located in Appendix 3.
Paint color and condition are noted and included in the data tables. Under Paint Condition an "I" indicates intact or sound paint; an "F" indicates fair condition (paint surfaces show minor damage); a "P" indicates poor
The report generation software combined the lead readings stored in the XRF analyzer with the test sample location information recorded in the field to produce data tables.
The transfer of data from the analyzer to the computer was made through an interface cable supplied as part of the XRF system.
condition (paint is peeling, cracking, or blistering or shows other significant damage).
4.0 LEAD-BASED PAINT DEFINITIONS
HUD considers paint to be lead-based paint (LBP) if the concentration is 1.0 milligram of lead per square centimeter (1.0 mg/cm2) or greater, or if the lead concentration is greater than 5000 parts per million (ppm) or 0.5% by weight. The Consumer Product Safety Commission (CPSC) currently considers paint to be lead-containing if the concentration of lead exceeds 600 ppm (0.06% by weight). In 1978, the CPSC banned the sale of lead-based paint to consumers and its application to areas where consumers have direct access to painted surfaces. This survey classified LBP on the basis of the HUD 1.0 mg/cm criteria and 0.5% by weight (5000 ppm).
By contrast, the mission of the Occupational Safety and Health Administration (OSHA), with respect to lead in paint, is to protect workers during construction activities that may generate elevated airborne lead concentrations. OSHA states that construction work (including renovation, maintenance, and demolition) carried-out on structures coated with paints that have lead concentrations lower than the HUD or CPSC limits can still result in airborne lead concentrations in excess of regulatory limits. For this reason, OSHA has not defined lead-containing paint, but states that paint having any measurable level of lead may pose a substantial exposure hazard during construction work, depending upon the work performed. The owner or employers must be aware of the OSHA Lead in Construction Standard (29 CFR 1926.62) and the Respiratory Protection Standard (29 CFR 1910.134).

According to HUD Guidelines, lead-based paint was identified at or above 1.0 mg/cm2 on interior and exterior building components sampled during the survey. The XRF Results Tables located in Appendix 3 identify the room number, building component, paint color and condition, and approximate square footage of lead-based paint materials.
Detailed Cost Estimates (Appendix 3) were developed for the removal of lead based paint building components. The Detailed Cost Estimates were derived by abatement professionals utilizing current industry costs for the current period (2012). All applicable taxes and markup are included.
This estimate is for abatement contractor plus Environmental Professional costs. Costs for the replacement of building materials that have been removed are NOT included.
The total estimated cost for the chemical removal of lead based paint at the GIMC Service Unit – Gallup Hospital Installation is $28,949.28. The total estimated cost for the component demolition of building materials applied with lead based paint at the GIMC Service Unit – Gallup Hospital Installation is $17,776.77.
6.0 DISCUSSION AND RECOMMENDATIONS
Lead-based paint (LBP) was detected on interior and exterior building components during sampling conducted by iiná bá) on January 11 to 17, 2012. The interior and exterior LBP components were in good condition. Due to the observed good condition of the LBP components, iiná bá) does not recommend abatement or maintenance attention to the LBP unless they will be disturbed by renovation or demolition activities.
The mere presence of LBP in a building poses no health hazard so long as the paint remains intact. Lead is only a hazard to adults and workers when it is inhaled, as fine dust or fume, or when it is ingested, when a painted surface is mouthed or chewed by a child, or inadvertently through contamination of surfaces, hands, and food by lead-contaminated dust. Therefore, to minimize the potential hazards associated with the presence of LBP, the following procedures should be implemented.
• Identification (through testing) of all known or suspect LBP surfaces and communication of the presence of LBP surfaces to workers potentially affected by the presence of LBP. Although the LBP testing has been completed, completion of a specific Operations and Maintenance (O&M) Program is suggested to organize a coherent, continuous program outlining procedures for routine inspections, maintenance, and immediate clean-up of damaged LBP surfaces. The O&M Program should address periodic inspections, maintenance activities, methods of safe and prompt clean-up when a licensed lead abatement contractor is not used, work procedures which are likely to create lead hazards, and a program of standard work procedures which are designed to minimize lead contamination. Development of an O&M Program was not part of iiná bá’s original Scope of Work. The on-site maintenance should perform periodic inspections to assess the condition of LBP (i.e.
chip, peel, or flake). If areas of LBP become damaged or deteriorated, a licensed lead abatement contractor should be contacted regarding removal or abatement.
Maintenance activities, which should be avoided in areas where known or suspect LBP is located, include, but are not limited to:
• Scraping, sanding, or using a heat gun on painted surfaces before repainting;
• Making holes in walls to access at pipes, install electrical outlets, etc.;
• Removing building components for renovation activities;
• Repeatedly bumping furniture or other objects into painted components;
• Unnecessarily opening and closing windows with LBP components. Major work or renovation performed on LBP components should be completed by a licensed LBP abatement contractor. However, if minor repair or immediate clean-up is necessary for a specific LBP component, at a minimum, the following work and clean-up procedures should be incorporated:
• Move children and pregnant women to another area of the facility until work is completed and the area is properly cleaned;
• Cover exposed areas using plastic to seal off entrances and ducts and protect furniture, carpets, rugs, and floors from paint dust and chips. Plastic should be carefully disposed;
• Wet the painted surface with soapy water prior to working;
• Thoroughly clean area following work. Clean-up dust and chips with wet mops or rags soaked in a solution of trisodium phosphate (TSP) or phosphate containing powdered dishwashing detergent. Use two buckets of water – one for wash and one for rinse. To prevent recontamination of cleaned surfaces, wash mops and rags thoroughly after each use. Dust, chips, debris, mops, and rags should be contained in plastic bags and disposed of properly by a licensed lead abatement contractor;
• Avoid sweeping or vacuuming the work area. Sweeping and vacuuming spread lead dust particles, since the filters are not fine enough to contain LBP particles;
• If an entire LBP component (such as a window sill or door frame) requires removal, a representative sample should be taken of the LBP-coated component and this sample should be analyzed via the Toxic Characteristic Leaching Process (TCLP) method to determine whether the component must be disposed of as a hazardous waste. A licensed lead contractor should take the sample and a certified, licensed laboratory should provide the analysis for the TCLP method.
There are three general control strategies for controlling lead paint. These are component replacement, encapsulation, and paint removal. The most cost effective up front control strategy is to encapsulate LBP by repainting areas of chipped, damaged, or deteriorating paint. Cleaning of the component and surface using procedures described above should be completed prior to encapsulation activities. Safe and prompt clean-up of LBP debris or dust when damage or deterioration is encountered should be completed by a licensed lead abatement contractor when possible. LBP component removal and paint removal are considered LBP abatement and would need to be completed by a licensed lead abatement contractor. Should removal or abatement be completed, an exposure assessment should be performed during the work, as required by the OSHA lead-in-construction standard.
iiná bá recommends personnel responsible for conducting maintenance of LBP components in the facility meet the training requirements for maintenance workers on LBP hazards. This training includes procedures that minimize lead dust generation and soil contamination during maintenance activities, as defined by the EPA and HUD.
Portions of the OSHA 29 CFR 1910.1025 General Lead Industry Standard apply to maintenance activities that involve making or keeping a structure, fixture, or foundation in proper condition in a routine, scheduled, or anticipated fashion.
7.0 PROJECT LIMITATIONS
This Project was performed using, as a minimum, practices consistent with standards acceptable within the industry at this time, and a level of diligence typically exercised by industrial hygiene and environmental consultants performing similar services.
The procedures used in this investigation attempt to establish a balance between the competing goals of limiting investigative and reporting costs and time, and reducing the uncertainty about unknown conditions. Therefore, because the findings of this report were derived from the scope, costs, time, and other limitations, the conclusions should not be construed as a guarantee that all environmental or occupational hazards have been identified and fully evaluated. Where sample collection and testing have been performed, iiná bá’s professional opinions are based in part on the interpretation of data from discrete sampling locations that may not represent conditions at non-sampled locations. Iiná bá assumes no responsibility for omissions or errors resulting from inaccurate information, or data, provided by sources outside of iiná bá or from omissions or errors in public records.
Furthermore, it is emphasized that the final decision on how much risk to accept always remains with the client since iiná bá is not in a position to fully understand all of the client's needs. Clients with a greater aversion to risk may want to take additional actions while others, with less aversion to risk, may want to take no further action.
