EVALUATION INSTRUCTIONS
The Offeror (Prime) shall list up to three (3) contract or subcontract references from current or prior customers that involve work of the same size, scope, and complexity that is being solicited by the TSA in this RFP. At a minimum, one (1) of the three (3) referenced contracts must be in support of a contract or order in which the Offeror was the Prime Contractor and not a sub-Contractor. Offerors that are newly formed entities without prior contracts should list contracts and subcontracts in which key personnel proposed for this effort were directly involved. "Recent" is defined as work having been performed within the past five (5) years.
For the “Past Performance” portion of the proposal, the Offeror shall identify the following information for each of the referenced contracts:
A. Name of Agency B. Primary Point of Contact (name, title, phone, and email) C. Alternate Point of Contact (name, title, phone, and email) D. Contract/Task Order Number E. Contract/Order Type F. Period of Performance G. Total Contract Dollar Value H. A detailed description of the scope/work performed for the contract.
I. A detailed explanation demonstrating the similarity of the contract to the requirements of this solicitation.
J. Indicate whether or not there has been a final Contractor Performance Assessment Rating (CPAR) for the contract.
The TSA may contact those references during the evaluation process to verify relevant experience and level of performance. The TSA may, at its discretion, obtain and evaluate information from sources other than those provided by the Offeror.
The Offerors are reminded that both independent data and data provided by the Offerors in their proposal may be used to evaluate past performance. Since the Government may not interview all sources provided by the Offerors, it is incumbent upon the Offeror to explain the relevance of the data provided. Information may also be considered regarding past performance of any significant sub-Contractor. The Government may use other information available from other sources to evaluate a Offeror’s past performance. The Government reserves the right to limit or expand the number of references it decides to contact and to utilize information other than that provided by the Offeror. The Offeror must provide the information requested, or affirmatively state that it possesses no past performance.
Submit completed Cover Sheet and Questionnaire by time and date to be designated in Advisory Notice that will be provided by TSA.
