About this file
This is a Campaign Contribution Disclosure Form required by the Incorporated County of Los Alamos Procurement Division for prospective contractors bidding on the Pharmacy Inspection Services contract. The form mandates that all prospective contractors disclose any campaign contributions totaling over $250 in the aggregate made by themselves, family members, or representatives to applicable public officials during the two years prior to proposal submission. The applicable public officials for this procurement are the seven County Councilors: Theresa Cull, Melanee Hand, Susie Havemann, Ryn Hermann, Beverly Neal-Clinton, David Reagor, and Randal Ryti. The form requires detailed reporting of contribution dates, amounts, nature, and purpose, with contractors required to certify either that contributions over $250 were made and disclosed or that no such contributions were made.
The form establishes comprehensive definitions for key terms including "applicable public official," "campaign contribution," "prospective contractor," "family member," and "representative of a prospective contractor." Contractors must submit this disclosure form regardless of whether they have made any reportable contributions. The form emphasizes that campaign contributions include gifts, subscriptions, loans, advances, deposits of money, and in-kind contributions made for the purpose of electing officials to statewide or local office, while specifically excluding volunteer services and unreimbursed personal expenses. Submission of this form is mandatory for all bidders participating in the competitive sealed proposal process for the Los Alamos County pharmacy inspection services contract.
