About this file

This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form required by the Incorporated County of Los Alamos Procurement Division in New Mexico for the Pharmacy Inspection Services solicitation. The form is a mandatory certification document that offerors and bidders must complete and submit as part of their proposal or bid package. Bidders are required to certify their eligibility to participate in covered transactions by attesting to their compliance with various responsibility standards and legal requirements.

The certification requires offerors to confirm that neither they nor any of their principals are currently debarred, suspended, proposed for debarment, or excluded from covered transactions by any federal, state, or local agency. Offerors must also certify that within the preceding three-year period they have not been convicted of fraud, criminal offenses related to public contracts, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records. Additionally, bidders must attest that they have not violated contract provisions, failed to perform satisfactorily, violated ethical standards under the County Procurement Code, or had public contracts terminated for cause or default. The form also requires certification that the offeror is not a current or former County employee and is not an immediate family member of a County employee or public official. Any inability to certify these statements must be accompanied by a detailed written explanation. Falsification of any statement in the certification constitutes grounds for rejection of the proposal or rescission of a contract award.
