About this file
This is an evaluation scoresheet for a Request for Proposal (RFP) issued by Box Elder School District in Utah for garbage disposal and waste removal services. The solicitation requires vendors to submit proposals by 2:00 p.m. on March 17, 2026, with questions due by 2:00 p.m. on March 11, 2026. Proposals must be submitted in five hard copies to Box Elder School District, Attn: Corey Thompson, 960 South Main Street, Brigham City, UT 84302. All inquiries should be directed to Corey Thompson at corey.thompson@besd.net. The evaluation process includes both mandatory prerequisites and scoreable technical criteria, with the final award decision based on cumulative scoring across multiple evaluation categories.
The evaluation scoresheet establishes a structured scoring methodology with three mandatory requirements that must be met before technical evaluation: submission of completed Attachment C (Cost Sheet), completed Attachment D (Technical Criteria), and container pricing information. Scoreable technical criteria are weighted equally at 20 points each across four categories: vendor experience and capability to meet agency requirements, pickup schedules and emergency fulfillment processes, submission and verification of three reference letters, and optimization of container sizes and pickup schedules to meet school district needs. A low-cost option component worth 20 points is evaluated based on the cost fee proposal provided in Attachment C. The total possible points from technical criteria and cost are calculated to determine final vendor ranking and contract award.
