About this file

This is an RFP evaluation scoresheet for Solicitation #NS26-156, issued by the State of Utah Division of Purchasing for the procurement of frozen and cooler food products through a statewide cooperative agreement. The RFP seeks vendors capable of providing bulk purchasing of frozen and cooler foods including deli meats, proteins, vegetables, beverages, seafood, bakery prepared meals, entrees, and desserts to state entities across Utah. The contract term is five years, with vendors required to guarantee pricing for an initial three-month period. The evaluation process is structured in two stages: vendors must first satisfy all mandatory minimum requirements, including submission of a completed cost sheet (Attachment C), evaluation criteria documentation (Attachment D), and lobbying disclosure forms, before proceeding to technical evaluation.

The technical evaluation comprises 700 points distributed across five scoring sections: Operational Approach and Deliverables (120 points), Logistics and Statewide Distribution (130 points), Ordering Systems and Product Data (210 points), Risk Management and Product Recalls (120 points), and Continuity of Operations and Disaster Plans (120 points). Vendors must achieve a minimum of 455 points (65%) in technical evaluation to advance to cost evaluation. The cost evaluation comprises 300 points, with the lowest-priced proposal receiving 100% of available cost points and other proposals receiving proportional points based on their price relative to the lowest bid. A total of 650 points out of 1,000 possible points (combining technical and cost scores) is required to receive a contract award. An administrative fee of 0.75% applies to all orders, and the RFP may result in multiple awards based on vendor qualifications and performance against these criteria.
