About this file

The document is a Litigation History Form for a state and local contract opportunity issued by Broward County, Florida, for Occupational Medical Services and Drug & Alcohol Testing. The open-end contract is designed to provide medical services and testing programs for the Risk Management Division and other county agencies. Vendors are required to disclose any material litigation cases from the past three years, including legal actions related to similar work, allegations of fraud or negligence, contract performance issues, financial conditions, or criminal proceedings. The solicitation requires electronic submission through BPRO, with vendors encouraged to submit responses in advance of the closing date.

The contract opportunity includes procurement preferences for Small Business Enterprises (SBE) and County Business Enterprises (CBE). Vendors must submit questions and clarification inquiries through BPRO by a specified date, with the county responding through the platform's messaging section. The litigation history form is a critical component of the vendor evaluation process, with failure to disclose material cases potentially resulting in a non-responsive designation. Vendors must provide comprehensive details about any material legal cases, including case numbers, court information, disposition, and relevant documentation.
