About this file
This document is an Anti-Human Trafficking Affidavit for Broward County, Florida, specifically related to an Occupational Medical Services and Drug & Alcohol Testing contract. The affidavit requires vendors to affirm they do not use coercion for labor or services as defined in Section 787.06, Florida Statutes, and must be submitted with the vendor's proposal or within three business days of the county's request. The associated contract opportunity is an open-end procurement for occupational medical services and drug and alcohol testing programs for the Broward County Risk Management Division and potentially other county agencies.
The solicitation emphasizes electronic submission through BPRO, with vendors required to submit responses electronically and obtain a Submission Receipt. The contract includes procurement preferences for Small Business Enterprises (SBE) and County Business Enterprises (CBE), and provides a specified period for vendors to ask questions and seek clarification about the solicitation requirements. Vendors must follow specific submission guidelines, including immediately notifying the Purchasing Agent and contacting technical support if experiencing difficulties with electronic submission. The affidavit must be signed under penalties of perjury, with spaces for vendor name, signature, printed name, title, and date.
