About this file

This document is a Domestic Partnership Act Certification form for Broward County, Florida, specifically designed for vendors participating in county solicitations. The certification is required for contract opportunities over $100,000 and mandates that vendors either currently provide or commit to providing benefits to registered domestic partners of employees equivalent to those provided to employees' spouses. Vendors must complete and submit this form with their bid, with a deadline of three business days after the county's request, and failure to do so may result in being deemed non-responsive to the solicitation.

The certification form includes multiple compliance options, such as currently offering domestic partnership benefits, committing to offer such benefits at contract award, or claiming exemption under specific conditions like employing fewer than five employees, being a governmental or non-profit entity, or having limitations based on federal or state laws. The form is part of a broader solicitation for Occupational Medical Services/Drug & Alcohol Testing for Broward County's Risk Management Division, with an emphasis on procurement preferences for Small Business Enterprises (SBE) and County Business Enterprises (CBE). Vendors are required to submit their responses electronically through the BPRO system and must follow specific guidelines for questions, clarifications, and submission procedures.
