About this file

The document is a 7-Item Expression of Interest (EOI) Open Enrollment Questionnaire from Milwaukee County Behavioral Health Services (BHS) Comprehensive Community Services (CCS) program, seeking to expand its network of ancillary service providers for youth and adults in 2026. The EOI is designed to identify new community providers and current providers looking to expand their service offerings, with specific services open to providers who can serve targeted participant groups. The questionnaire covers a comprehensive range of topics including service descriptions, existing programming, evidence-based practices, outcome data, client satisfaction, agency mission, linguistic and cultural capacities, experience, licensure, staff qualifications, and operational capabilities.

The EOI focuses on providers who can deliver services within Milwaukee County, with requirements including face-to-face service delivery, adherence to clinical supervision standards outlined in DHS 36.11, commitment to initial and ongoing training requirements, and ability to demonstrate competence in serving behavioral health populations. The document emphasizes multidisciplinary team approaches, cultural competency, and the use of informal and natural supports. Current CCS network providers seeking to continue existing services are explicitly instructed not to use this EOI process, which is specifically targeted at new providers or those seeking to expand their service offerings in the 2026 program year.
