About this file
This document is an Additional Disclosures/Certifications form for Milwaukee County Behavioral Health Services (BHS) Comprehensive Community Services (CCS) program, related to an Expression of Interest (EOI) for ancillary service providers. The EOI seeks to expand the CCS Network by identifying new community agencies and organizations interested in providing services for youth and/or adults in 2026, with specific opportunities for both new and current providers to add or expand service offerings.
The form includes critical compliance and background disclosure sections requiring respondents to detail any past contract performance issues, legal proceedings, audit findings, criminal history, and commitment to specific operational standards such as HIPAA compliance, background checks, and data system compatibility. Providers must disclose any historical issues that might impact their eligibility, including contract terminations, legal violations, audit findings, or criminal backgrounds of principals. The EOI is specifically targeting providers who can serve targeted participant groups and are interested in expanding or entering the Milwaukee County CCS service network for the upcoming contract period.
