About this file

This is a Minimum Insurance Requirements document for a Microfilm Conversion Scanners & Software project administered by Broward County's Libraries Division in Florida. The project involves the purchase and delivery of two microfilm conversion scanners and associated software designed to digitize microfilm images automatically. The contract scope includes installation, training, maintenance, and on-site service support. The certificate holder for insurance purposes is Broward County, located at 115 South Andrews Avenue, Fort Lauderdale, Florida 33301, with oversight provided by the Risk Management Division.

The insurance requirements establish comprehensive coverage obligations for the contractor. General Liability coverage must include a minimum of $1,000,000 per occurrence and $2,000,000 aggregate with broad form protection covering premises operations, explosion/collapse/underground, products/completed operations, contractual liability, and independent contractors. Auto Liability coverage of $1,000,000 combined bodily injury and property damage is required if applicable to project performance. Professional Liability/Technology Errors & Omissions coverage must provide $1,000,000 per claim with a maximum deductible of $100,000. Excess Liability/Umbrella coverage may supplement minimum requirements. Workers' Compensation at statutory limits and Employers Liability with $500,000 per accident coverage are mandatory. Broward County must be named as an additional insured, receive primary coverage without contribution requirements, and benefit from a waiver of subrogation. For claims-made policies, insurance evidence must be maintained for three years following contract completion.
