About this file

This is a Vendor Questionnaire form for Broward County, Florida, applicable to a solicitation for microfilm conversion scanners and software for the Libraries Division. The questionnaire must be completed and submitted with the initial solicitation response, or within three business days of the County's written request, with failure to submit timely resulting in the vendor being deemed nonresponsive. The project requires the purchase and delivery of two microfilm conversion scanners and associated software, along with installation, training, maintenance, and on-site service support. The contract scope encompasses digitization of microfilm images through automated scanning processes.

The questionnaire requires vendors to provide standard business information including legal name, FEIN, office location, contact details, and authorized representatives. Vendors must affirm they are not debarred or suspended by any government entity within three years; certify authorization as a dealer or seller with manufacturer warranty; confirm willingness to extend identical pricing to other governmental entities; and accept Visa payment without additional fees if the contract is approved for procurement card use. Vendors must complete non-collusion certifications, disclose any participation in solicitation development, and may qualify for preference if they certify implementation of a drug-free workplace program in accordance with Florida Statutes Section 287.087. All responses must be signed by an authorized representative under penalty of perjury, with false statements subject to rejection, award rescission, contract termination, or vendor debarment under the Broward County Procurement Code.
