About this file

The document is an Evaluation Score Sheet for a Disaster Cultural Restoration Request for Proposal (RFP) issued by the State of Utah Division of Purchasing. The RFP seeks to establish a contract for disaster response and recovery services for cultural collections, with the potential for multiple awards. The contract will have a five-year term, with vendors required to guarantee pricing for one year. The evaluation process involves a technical scoring system with a maximum of 800 points for technical criteria and 200 points for cost, with vendors needing to achieve a minimum of 550 technical points to advance to the cost evaluation stage and a total of 650 out of 1,000 points to be awarded a contract.

The RFP includes ten technical evaluation criteria, each weighted at 10%, covering areas such as response time, emergency planning, personnel screening, facility security, inventory management, stabilization, work documentation, safety programs, and charge resolution processes. The cost evaluation will use a formula that awards full points to the lowest-priced proposal, with other vendors receiving proportional points. An administrative fee of 0.50% is associated with the contract. The scoring methodology is structured to comprehensively assess vendors' capabilities in cultural disaster recovery services, with a rigorous point-based evaluation system that considers both technical expertise and pricing.
