About this file
This document is a Cost Sheet (Attachment C) for the State of Utah Division of Purchasing's Disaster Cultural Restoration contract opportunity. The procurement seeks contractors to provide disaster response and recovery services for cultural collections, with the potential for multiple contract awards. The contract will have a five-year term, with vendors required to guarantee pricing for one year. Contractors must meet a technical threshold of 550/800 points to proceed to cost evaluation, and ultimately must achieve 650/1000 total points to be awarded a contract.
The cost sheet requires contractors to complete hourly rates for specific labor positions, including Project Estimator, Project Manager, General Labor, Labor Foreman, and Hazmat/Asbestos Technician. The cost evaluation procedure will award 200 points based on pricing, with the lowest proposed price receiving 100% of cost points. Other contractors will receive a proportional share of points calculated by the formula: Cost Points x (Lowest Proposed Price/Proposed Price). An administrative fee of 0.50% will be applied to the contract, and contractors are allowed to list additional labor positions or fees that may be required for emergency project completion.
