Limited Asbestos Inspection and Lead-Based Paint Survey Report

This document is a Limited Asbestos Inspection and Lead-Based Paint Survey Report prepared by Hill West Environmental for the State of Utah Department of Administrative Services, Division of Facilities Construction and Management (DFCM), regarding the Utah State Hospital Support Services Building located in Provo, Utah. The survey was conducted on July 21, 2026, in preparation for HVAC unit upgrades to the building. The inspection focused exclusively on areas that will be impacted during the renovation project, including examination of sheetrock wall systems and floor tiles. Hill West Environmental collected bulk samples of suspect asbestos-containing materials and conducted lead-based paint inspections using X-ray Fluorescence (XRF) analysis with a detection threshold of 0.4 mg/cm². The survey identified one asbestos-containing material: pipe thread sealant with 2% chrysotile content measuring approximately 129 square feet, with an estimated abatement cost of $5,000. All other tested materials, including new and old roof cores, various caulks, and flashing tapes, tested negative for asbestos. Lead-based paint testing found no positive readings above the detection threshold in any tested materials. No universal, hazardous, or toxic wastes were identified as being impacted by the renovation project.

The report was prepared by Claude Dahlk, CHMM, CIEC, an accredited asbestos inspector holding certification number ASB-0433 with Hill West Environmental (State Certification # ASBC-495). Analytical testing was performed by Eurofins Aerotech Built Environment Testing, LLC, an NVLAP-accredited laboratory. The survey supplemented a previous 2022 limited asbestos survey conducted by R&R Environmental, which had identified the same pipe thread sealant as containing 2% chrysotile asbestos. The report emphasizes that if contractors discover additional suspect building materials during renovation activities that were not included in this survey, work must be halted and DFCM must be notified for further sampling and analysis. All findings and conclusions are based solely on the samples analyzed and the limited scope of inspection restricted to renovation impact areas, with no warranty provided regarding the identification of all asbestos or hazardous materials that may exist elsewhere within the facility or in areas not inspected.

EXECUTIVE SUMMARY

On July 21, 2026, Hill West Environmental conducted a limited pre-renovation inspection for asbestos and lead-based paint of the Utah State Hospital - Support Services Building located at 1300 East Center Street, Provo, Utah 84606. The project includes upgrades to the HVAC Units within the project areas.

Ms. Aubrey Stapel, Project Manager with the State of Utah, Division of Facilities Construction and Management (DFCM) requested this inspection to identify asbestos-containing materials (ACM) and building components with lead-based paint that need to be addressed before the building is renovated.

This building was previously sampled for a roofing upgrade with the old roof being replaced at that time. With the pipe thread sealant testing positive for asbestos and the old roof being none detected for asbestos. A copy of this report is attached in Appendix B of this report.

Suspect materials sampled during the 2022 and this inspection included the new roof core, pipe thread sealant, tape, and caulking. Areas in which renovations are to occur were inspected, and all other areas were not inspected as part of this survey.

Hill West has identified the following asbestos-containing materials (ACM) and lead-based paint during this inspection:

Conclusions

ACM – Asbestos Containing Materials (ACM) were identified within the pipe thread sealant as identified in the 2022 report with all other materials testing negative for asbestos. These materials are regulated by the Occupational Safety and Health Administration (OSHA) and should be properly managed for abatement if these materials are to be impacted during renovation activities.

If the renovation or demolition contractor discovers additional suspect building materials that were not sampled as part of this inspection, they shall stop working and notify DFCM so that the materials can be sampled.

Lead – Based on the 2022 report, lead was not detected in any of the materials tested on the roof of this structure. If the renovation or demolition contractor discovers additional suspect building materials that were not sampled as part of this inspection that will be impacted, they shall stop working and notify DFCM so that the materials can be sampled.

Universal, Hazardous and Toxic Wastes – No Universal, Hazardous, and Toxic Wastes will be impacted during this renovation project.

1.0 INTRODUCTION

As per DFCM’s instructions, Hill West Environmental conducted a limited asbestos and lead-based paint inspection of the Utah State Hospital - Support Services Building located at 1300 East Center Street, Provo, Utah 84606. Due to the remodel and upgrades to the HVAC Unit upgrades, Hill West was instructed to only inspect areas that will be impacted during the renovation project.

The purpose of this survey was to identify the existence, extent, and condition of both friable and non-friable asbestos-containing materials (ACM) and lead-based paint/lead materials within the renovation areas.

2.0 SITE VISIT /OBSERVATIONS

2.1 Asbestos Inspection

On July 21, 2026, Hill West Environmental visited the site to survey the renovation areas of the building. A complete asbestos inspection of the areas that will be impacted during renovations was made to determine conditions and develop a sampling scheme for the areas. The materials that will be impacted during this upgrade project include sheetrock wall system and floor tile. Table 1 summarizes the asbestos inspection results.

2.2 Lead-based Paint Inspection

On April 21, 2022, R&R Environmental conducted a limited lead-based paint inspection of the Utah State Hospital – Support Services Building on equipment and other painted surfaces located on the roof. Direct measurements of lead in paint/dust were made using a Niton XLp 300 X-ray Fluorescence (XRF) Spectrum Analyzer. The Niton XRF non-destructively measures lead concentrations of painted surfaces, regardless of the number of layers present.

According to the manufacturer, the detection limit (lower limit of reliable measurement) for this instrument is 0.1 milligrams per square centimeter (mg/cm2) ±0.3 mg/cm2 with the instrument set on the “quick” measuring mode.

The quick mode provides 95% confidence that the lead concentration in the paint is above or below the set point of the instrument which, for this survey is 0.4 mg/cm2. For this survey, measurements below 0.4 mg/cm2 were not reported as lead containing.

The Niton XRF sometimes reports negative values. According to the manufacturer, negative values should be expected and interpreted as zero lead content due to the statistical variability of XRF measurement technology. Both HUD and the EPA recognize the statistical variability of XRF technology and the possibility of obtaining negative values where the lead content is near zero.

Table 2 summarizes the lead-based paint inspection results. Measurements are reported in milligrams of lead per square centimeter (mg/cm2). All the coatings tested during this inspection were in good to fair condition. Only positive readings (>0.4 mg/cm2) are summarized in Table 2. Appendix B contains all XRF readings for this survey.

4.0 DISCUSSION/CONCLUSIONS

In Utah, EPA asbestos regulations are administered by the Utah Division of Air Quality. Utah Occupational Safety and Health Administration (UOSHA) has adopted the Federal OSHA regulations. In addition, the Salt Lake Valley Health Department (SLVHD) regulates demolition activities in Salt Lake County. The SLVHD regulations for pre-demolition building inspections require an asbestos inspection, but also required building owners to inspect the building for other hazardous materials such as universal wastes, hazardous and toxic wastes and lead-based paint. Similar to asbestos, these wastes, if present must be removed prior to demolition/renovation.

A copy of this report should be on-site during all renovation activities.

This limited asbestos and lead-based paint inspection was conducted on the project area that will be impacted during renovation activities. Only items that are believed to be impacted during renovation activities were inspected during this survey. This survey is not intended to represent an exhaustive research of all asbestos or other hazards or conditions which may exist within other buildings located at this project site.

This report does not purport to represent future indoor conditions or events.

Situations or activities which transpire subsequent to this report, which result in adverse environmental, construction and/or engineering impacts, are not to be construed as relevant to this study.

This report is intended for the sole use of Hill West's client. This report may not be used or relied upon by any other party without the written consent of Hill West.

The scope of services performed in execution of the evaluation may not be appropriate to satisfy the needs of other users, and the use or re-use of this document or the findings, conclusions, or recommendations is at the risk of said user. 3. The Environmental Protection Agency (EPA) National Emission Standard for Hazardous Air Pollutants

(NESHAP) asbestos revision as outlined in 40 CFR, Part 61, became effective November 20, 1990. The asbestos classification system outlined in the revision and included in this section is dynamic in nature.

Asbestos materials classified as “NON-REGULATED” at the time of the survey may become “REGULATED” due to ongoing or planned maintenance, renovation, or demolition actions, which can transform a material containing greater than 1% asbestos from a “non-friable” and NON-REGULATED to a friable and REGULATED condition. Classification of ACM in this section and in the executive summary of this report is, therefore, based on the observations of the surveyor at the time of the survey and may or may not be appropriate at later dates.

4. Maintenance, renovation, demolition, weathering, normal wear, water or other damage can alter the “NON- REGULATED” status of materials, and necessitate precautions required for handling them as “REGULATED” asbestos-materials.

Support Services Building Roof Page 5

3.0 SAMPLING METHODS

Survey procedures were based on those outlined in Title 40 Code of Federal Regulation (CFR) Part 763, USEPA Asbestos Hazard Emergency Response Act, and the Asbestos Model Accreditation Plan (effective April 4, 1994). The procedures included visual observation, physical inspection, bulk sample collection, and condition assessment of suspect ACM. The number of samples collected was based on the amount and accessibility of each homogeneous material, with consideration given for the type, age, and condition.

Nineteen (19) samples were collected from the suspect materials to assess the presence of asbestos (or lack thereof) in building materials/systems. Each sample collected was placed into a sealable plastic bag or tub, marked with a unique sample identification code, and packaged for subsequent shipment to the laboratory for analysis. Individual sample identification consisted of a numeric code denoting the sampling media/location.

Subsequent to packaging and labeling, samples were submitted under proper chain-of-custody to Reservoirs Environmental, Inc., Denver, Colorado. The National Voluntary Laboratory Accreditation Program (NVLAP), Lab Code 101896 accredits Reservoirs for asbestos analysis. Samples were analyzed via Polarized Light Microscopy (PLM) utilizing EPA Method 600/M4-82-020. Please note that USEPA and OSHA regulations define an ACM as a material containing more than one (1) percent asbestos, by volume, as determined by PLM analysis. In the case of multi-layer systems (e.g., floor tile/mastic layers), the laboratory analyzed and reported each layer separately.

4.0 SURVEY RESULTS

Nineteen (19) representative samples collected were submitted for analytical determination. A summary of analytical results for samples collected and analyzed for this survey are in Appendix B (Laboratory Results) and are discussed below by building material/system type. Photographs of the buildings where samples were taken are included as Appendix C. Floor Plans of the building where samples were taken are included in Appendix D

Support Services Building Roof Page 6

5.0 CONCLUSIONS

Asbestos-containing materials were identified in the building. See Executive Summary for details.

6.0 LIMITATIONS AND EXCLUSION OF WARRANTY

This asbestos survey and assessment was performed using procedures and a level of diligence typically exercised by professional consultants performing similar services. However, asbestos-containing material (ACM) can be present in a structure, but not identified using ordinary investigative procedures.

No asbestos survey can completely, eliminate uncertainty regarding the presence of ACM. R & R Environmental, Inc.’s level of diligence and investigative procedures are intended to reduce, but not eliminate, potential uncertainty regarding the presence of ACM. The procedures used for this survey attempt to establish a balance between the competing goals of limiting investigative costs, time, and building damage, and reducing the uncertainty about unknown conditions. Therefore, the determinations in this report should not be construed as a guarantee that all ACM present in the subject property has been included in this report.

This report presents R & R Environmental, Inc.’s professional determinations, which are dependent upon information obtained during performance of consulting services. R & R Environmental, Inc. assumes no responsibility for omissions or errors resulting from inaccurate information provided by sources outside of R & R Environmental, Inc.

No warranty or guarantee, expressed or implied, is made regarding the findings, conclusions, or recommendations contained in this report. The limitations presented above supersede the requirements or provisions of all other contracts or scopes of work, implied, or otherwise, except stated or acknowledged herein.

Limited Lead-Based Paint Inspection

On April 21, 2022, a limited lead-based paint (LBP) survey was conducted for the Utah State Hospital Support Services Building Roof. The purpose of the survey was to identify lead in paint on exterior surfaces of the building. Measurements for lead in paint were made using a Niton XLp 300 X-ray Fluorescence (XRF) Spectrum Analyzer. No chip sampling or laboratory analysis was performed for confirmation of XRF measurements.

The survey work was overseen by David Roskelley with R & R Environmental, Inc. in Sandy, Utah. David Roskelley has completed Lead Inspector Training through the University of Utah, Rocky Mountain Center for Occupational and Environmental Health (RMCOEH), an EPA-sponsored Regional Lead Training Center, and is certified by the State of Utah, Division of Environmental Quality, as a Lead Inspector.

The U.S. Department of housing and Urban Development (HUD) Guidelines for the Evaluation and Control of Lead-Based Paint Hazards in housing (HUD Guidelines), Chapter7: Lead-Based Paint Inspection, 1997 Revision, was generally followed for this survey, with modifications appropriate for a non-residential building.

The following accredited and certified inspector oversaw the inspection, collected the samples and made assessment:
