Status 1: The utilities are in conflict with the project and REQUIRE relocation by OTHERS DURING construction.

Status 2: The utilities are in conflict with the project and REQUIRE relocation by the Contractor DURING construction.

Status 3: The utilities are in conflict with the project and REQUIRE relocation BEFORE construction.

Status 4: The utilities are located within the project rights of way but require NO relocation.

Wellton-Mohawk Irrigation and Drainage District: There are existing overhead power lines within the project limits that cross the existing road, but they do not interfere with the proposed improvements. Protect in place all existing power poles adjacent to the existing roadway.

Century Link Communications: There are direct buried communication facilities under the roadway prism. As no excavation under the roadway prism is necessary with the obliteration of the existing roadway, no conflicts are anticipated.

For all Utilities: Within 30 days of the kick-off meeting, pothole the locations where utility conflicts are anticipated. Provide utility verification potholing for the direct buried communication lines at a minimum. Protect all utilities in place.

Comply with all applicable provisions of Arizona State Law when planning or performing excavations at utility test hole sites. Compliance actions include, but are not limited to: notify owners or operators of underground utility facilities at least two (2) business days prior (not including the day of actual notice) to making or beginning excavations in the vicinity of such

Red Cloud Mine Rd Yuma County, AZ facilities; call USA Locate (One-Call Utility Locate Service) for the marking of member utilities;

contact non-member utilities directly; coordinate with utility owner representatives as required for inspection or other on-site assistance.

Immediately cease excavation work and report any resultant utility line damage to the owner, should damage occur.

Coordinate with the utility company representatives. At the preconstruction conference, designate a contact that will be responsible for coordinating all utility work with the utility companies. Do not designate the Superintendent as the utility contact person. Complete and supervise all temporary and permanent utility work with experienced and qualified personnel within the field of that particular utility being worked on. Submit for review and approval documentation and certification to verify that the personnel working on the particular utility are qualified to work on that specific utility. Notify all utility companies and the CO a minimum of ten (10) business days prior to work in the vicinity of utilities. The notification should be in a letter form and include a station to station listing of construction work. Prepare a utility service interruption contingency plan and provide to the CO at the preconstruction conference.

When locating and potholing utilities, use minimally intrusive excavation techniques that ensure the safety of the excavation, the integrity of the utility line to be measured, and that of other lines which may be encountered during excavation. Consider excavating by using air- or water-assisted vacuum excavation equipment manufactured specifically for this purpose.

107.05 Responsibility for Damage Claims. Delete the first sentence of the third paragraph and substitute the following:

Before work begins, submit “certificates of insurance” certifying that the policies will not be changed or canceled until 30 days written notice has been given to the Government.

107.10 Environmental Protection.

(a) Federal Water Pollution Control Act (Clean Water Act) 33 USC § 1251 et seq. Add the following:

(4) Do not ford running streams with construction equipment. Obtain approval from the CO to use temporary bridges or other structures whenever crossings are necessary.

(5) Immediately clear ephemeral drainages, intermittent and perennial streams, lakes and reservoirs of all work items, debris or other obstructions placed by or resulting from construction operations.

(6) Locate machinery servicing and refueling areas away from streambeds and washes to reduce the possibility and minimize the impacts of accidental spills or discharges.

(b) Oil and hazardous substances. Add the following to the end of the third paragraph:
