Except as provided herein, all terms and conditions of the document referenced in Item 9 A or 10A, as heretofore changed, remains unchanged and in full force and effect .

LIST OF CHANGES:

The purpose of the amendment is to remove a clause, revise Section L.3 (f), revise Attachment 10 (Past Performance Survey), revise Attachment 11 (Client Authorization Letter), add a Questions and Answers attachment.

FIRST: Due to a system issue, 3.6.1-17 North American Industry Classification System (NAICS) Code (JAN 2017) could not be removed. This clause is not applicable.

THIRD: Section L.3 Instructions for Preparation and Submission of Proposals (f) is hereby revised as follows:

FROM: (f) Section II - Past Performance Information The Offeror (General Contractor) is required to provide Past Performance Surveys (PPS), attached to the SIR (Section J, Attachment 10), along with Client Authorization Letters (format at Section J, Attachment 11) to past customer/clients. The Offeror will send Past Performance Surveys (PPS) to clients who are familiar with the work demonstrated through previous contracts of a similar nature. Only projects completed within the last seven (7) years will be considered; however, one (1) project that is currently in progress and at least 50% complete maybe considered for no more than one (1) PPS requirement for the Offeror. The PPS will be returned directly to the Contracting Officer. Offerors shall request a minimum of two (2) with a maximum of five (5) Past Performance Surveys as a general contractor. The Offeror (General Contractor) shall provide, at a minimum, the following information for each PPS primary reference for each project.

Note: Alteration and/or deletion of data fields contained in the PPS may result in the offeror being eliminated from further consideration.

Name of Responder Responder's Title Responder's Email address Responder's phone number Name of Organization Name of Contractor Contact Number Project Title Contract Type Contractor Discipline: General, Mechanical, Electrical, Pool Original contract completion date based on Notice to Proceed Actual Contract Completion Date Rational for variance between original completion date and actual completion date Original contract amount and final contract amount Rationale for variance between original contract amount and final contract amount Brief project summary in order to explain how the project is relevant to this WiWAVES Facility Information on if a deficiency notice was issue Information on if a cure notice was issued Information on if a show cause was issued Evidence (a copy of the sent email showing the date, time, and email address for the party who was sent the email) to show that the PPS was provided to the primary reference by no later than 10 days prior to the SIR closing date along with instructions to provide the completed PPS to the Contracting Officer by no later than the SIR closing date. This evidence shall be provided for all PPS for the Offeror.

The Offeror will complete Pate 1 of the PPS and then provide the surveys to their references who will in turn complete and return the surveys directly to the Contracting Officer by the SIR closing date.

CAUTION: Offerors are cautioned that the FAA may use the data provided by other sources in evaluating past performance. Offerors will be given an opportunity to rebut information considered negative and relevant to the evaluation. While the FAA may consider data from other sources, the burden of providing thorough and complete past performance data rests with the Offeror.

The Contracting Officer will review each offeror's PPS submission to ensure that the offeror has provided evidence (a copy of the sent email showing the date, time, and email address for the party who was sent the email) to show that the PPS, described in Provision L.3, Instructions for the Preparation and Submission of Proposals, was provided to the primary reference by no later than 10 days prior to the SIR closing date along with instructions to provide the completed PPS to the Contracting Officer by no later than the SIR closing date for a minimum of two (2) with a maximum of five (5) Past Performance Surveys as a general contractor. Failure of the offeror to timely provide this information will result in the offeror being eliminated from further consideration.

The Past Performance Surveys shall be evaluated utilizing the five level assessment rating of "Exceptional", "Very Good", "Satisfactory", "Marginal", or "Unsatisfactory", as defined below.

Rating Definition Exceptional Performance meets contractual requirements and exceeds many to the Government’s/Owner’s benefit. The category being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

Very Good Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The category being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

Satisfactory Performance meets contractual requirements. The category being assessed contains some minor problems for which corrective actions taken by the contractor appear to be or were satisfactory.

Marginal Performance does not meet some contractual requirements. The category being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.

Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The category being assessed contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

TO: (f) Section II - Past Performance Information The Offeror (General Contractor), as well as proposed Mechanical, Electrical, and Pool sub-contractors, are required to provide Past Performance Surveys (PPS), attached to the SIR (Section J, Attachment 10), along with Client Authorization Letters (format at Section J, Attachment 11) to past customers/clients. The Offeror, along with their Mechanical, Electrical, and Pool sub-contractors, will send Past Performance Surveys (PPS) to clients who are familiar with the work demonstrated through previous contracts of a similar nature. Only construction projects completed within the last seven (7) years will be considered; however, one (1) project that is currently in progress and at least 50% complete maybe considered for no more than one (1) PPS requirement for the General Contractor as well as for the Mechanical, Electrical, and Pool sub-contractors. The PPS will be returned directly to the Contracting Officer. Offerors shall request a minimum of two (2) with a maximum of five (5) Past Performance Surveys as a General Contractor, as well as a minimum of two (2) and a maximum of five (5) Past Performance Surveys from each of their Mechanical, Electrical, and Pool sub-contractors. The Offeror (General Contractor) shall provide, at a minimum, the following information for each PPS primary reference for each project.

Note: Alteration and or deletion of data fields contained in the PPS may result in the offeror being eliminated from further consideration.

Name of Responder Responder's Title Responder's Email address Responder's phone number Name of Organization

Name of Contractor Contact Number Project Title Contract Type Contractor Discipline: General, Mechanical, Electrical, Pool Original contract completion date based on Notice to Proceed Actual Contract Completion Date Rational for variance between original completion date and actual completion date Original contract amount and final contract amount Rationale for variance between original contract amount and final contract amount Brief project summary in order to explain how the project is relevant to this WiWAVES Facility Information on if a deficiency notice was issue Information on if a cure notice was issued Information on if a show cause was issued Evidence (a copy of the sent email showing the date, time, and email address for the party who was sent the email) to show that the PPS was provided to the primary reference by no later than 10 days prior to the SIR closing date along with instructions to provide the completed PPS to the Contracting Officer by no later than the SIR closing date. This evidence shall be provided for all PPS for the General Contractor as well as Mechanical, Electrical and Pool subcontractors.

The Offeror and the proposed Mechanical, Electrical, and Pool sub-contractors will complete Page 1 of the PPS and then provide the surveys to their references who will in turn complete and return the surveys directly to the Contracting Officer by the SIR closing date.

The Contracting Officer will review each offeror’s PPS submission to ensure that the offeror has provided evidence to show that the PPS, described in Provision L.3, Instructions for the Preparation and Submission of Proposals, was provided to the primary reference by no later than 10 days prior to the SIR closing date along with instructions to provide the completed PPS to the Contracting Officer by no later than the SIR closing date for a minimum of two (2) with a maximum of five (5) Past Performance Surveys as a General Contractor as well as a minimum of two (2) and a maximum of (5) Past Performance Surveys for each of their proposed Mechanical, Electrical, and Pool sub-contractors.

Questions and Answers

1. QUESTION: How is the evidence email to be submitted? Is it as simple as attaching the PDF copy of the emails to the submission email? Are they to be a second attachment or a part of the section 1 submission?

Does this effectively make it so that the only submission documents from GC’s for section 2 is proof of the sent emails as the responses will come directly to you?

ANSWER: The evidence provided can be the email submitted to the reference. It can be a PDF. In accordance with L.3 Instructions for Preparation and Submission of Proposals (f), the evidence should be provided with section 2. Please refer to L.3 Instructions for Preparation and Submission of Proposals for what should be submitted.

2. QUESTION: As a General Contractor, are we required to submit survey’s for Mechanical, Electrical, and

Pool Subcontractors on the projects we plan to submit a survey on?

ANSWER: The intent is for the General Contractor to have the subcontractors they intend to use on this project submit surveys as described in the SIR. Amendment 0001 has revised Section L.3 (f).

3. QUESTION: The sample client authorization letter is referenced as attachment 11 in Section J, however, the sample client authorization letter has attachment 12 listed in the bottom right. Please confirm this is intended to be attachment 11. On the same note, the contractor’s release is listed as attachment 12, however, no attachment number is listed on the document.

ANSWER: The sample client authorization letter has been revised and the updated copy can be found attached to Amendment 0001. The contractor’s release does not need an attachment number listed on the document.

4. QUESTION: Reference attachment 10, page 2, #2. Please confirm verbiage referencing “HQB Building Lobby Remodel” is incorrect. Will this document be corrected and reissued?

ANSWER: Attachment 10, page 2, #2 has been revised and the updated copy can be found attached to Amendment 00001.
