The purpose of this Amendment is to:

1) Provide an additional bridge inspection report for another bridge possibly on the hauling path.

2) and 3) Revise B-Pages Proposal Instructions to make required changes.

4) through 10) Revise Special Contract Requirements (J-Pages) to make required changes.

5) Respond to bidder questions.

6) Provide pre-proposal meeting minutes and the pre-proposal powerpoint presentation.

7) The proposal due date will be changed to January 18, 2022, at 2:00 pm EST.

Request for Proposal Changes

1) Add bridge inspection report for Structure number(s) 5600-089P.

2) Delete the third sentence of the first paragraph of Subsection 1.C.(a).4). of Bid Schedule

Instructions, page 2, and substitute the following:

The Senior Structural Engineer shall be a registered Professional Engineer.

3) Delete the first two sentences of the second paragraph of Subsection 1.C. of Bid Schedule

Instructions, page 4, and substitute the following:

Submit three (3) bound paper copies and one (1) unbound paper copy of the Technical

Proposal. In addition, submit one (1) electronic copy of the Technical Proposal either on a CD ROM or on a USB stick.

4) Delete the last sentence of the second paragraph of Subsection 104.05 of the Special

Contract Requirements, page J-3 and substitute the following:

Submit load rating calculations that are signed and sealed by a Professional Engineer registered in the USA according to Subsection 104.03(a)(1).

5) Delete the last sentence of the first paragraph of Subsection 575.03 of the Special

Contract Requirements, page J-47 and substitute the following:

Furnish bridges designed by a licensed professional engineer. The project is being permitted with the temporary bridges as shown in the plans. If the contractor proposes alternate designs for the temporary bridges that will result in any fill in the creek or wetlands or increased disturbance, this could require updated environmental permits. The contractor will be responsible for confirming the acceptable PE licensure requirements for permits. Delays due to licensure issues will not be approved.

6) Delete the first sentence of Subsection 586.04(a)(3) of the Special Contract

Requirements, page J-50 and substitute the following:

Provide a senior structural engineer who is responsible for design support during construction and is a registered Professional Engineer in any State or Territory of the

United States, including District of Columbia.

7) Delete the last sentence of Subsection 586.04(b) of the Special Contract Requirements, page J-50 and substitute the following:

In addition, show that the pre-casting plant has performed at least one precast segmental superstructure concrete box girder project in the past twelve years similar in scope to this project.

8) Delete Subsection 586.05(i) of the Special Contract Requirements, page J-53 and substitute the following:

(i) Calculations. Submit calculations signed and sealed by a Professional Engineer registered in the USA to show that the loads imposed on the permanent structure by the erection, equipment, and in the handling, stacking, or storage of precast segments, will not adversely affect the structural adequacy of the permanent structure or exceed allowed stresses during the construction process.

9) Delete the second paragraph of Subsection 586.08(b) of the Special Contract

Requirements, page J-55 and substitute the following:

Perform geometry control at the casting yard and at the bridge site under the direct responsible charge of a Professional Engineer or Land Surveyor registered in the USA.

10) Delete the Subsection 587.05(g) of the Special Contract Requirements, page J-71 and substitute the following:

(g) Calculations for post-tensioning signed and sealed by a Professional Engineer registered in the USA.

11) Please see below for the responses to questions submitted to the Government by email.

Question: In reference to the Bid Schedule Instructions Section 1.C. Technical Proposal

Submittal pages 4 &5 (or PDF Pages 14 & 15):

a. Does the Offeror’s Qualifications Form, provided with the bid documents, get included within and count towards the 15-page limit for the Technical Proposal?

b. The last paragraph in this Section requires (1) electronic copy on a CD-ROM. Can we provide (1) electronic copy on a USB instead of a CD-ROM?

c. The last paragraph also states “Submit (4) bound paper copies”; however, on Page 2 of the Request For Proposals (PDF Page 9) it states “Submit (3) bound paper copies”. How many bound paper copies will EFLHD require for submittal?

Response:

a) The completed Offeror’s Qualifications Form can be submitted within the same envelope containing the Technical Proposal, but it does not count towards the page limit.

b) Yes, an electronic copy on a USB is acceptable.

c) Three (3) bound paper copies are required. The RFP will be revised with this change.

Question: Page 2 of the Request for Proposals indicates that "three (3) bound paper copies, one

(1) unbound copy, (1) and one CD-ROM" of the Technical Proposal are required. Page 5 of the

Bid Schedule Instructions indicates four (4) bound paper copies, one (1) unbound paper copy, and one (1) electronic copy on CD ROM. Please clarify how many copies of the Technical

Proposal are required.

Response: Three (3) bound paper copies are required. The solicitation will be revised with this change.

Question: At the bottom of the Table of Contents, included at the beginning of the RFP (PDF

Page 3), the underlined statement in bold font shows the items within the table of contents that are to be included in the submittal package. Section K is not in bold font; however, Section K located at the end of the RFP contains several items that the Offeror must check. Since there are requests for responses, is Section K required to be included with the Proposal Submittal

Package?

Response: An Offeror does not need to complete and return Section K if all of the Offeror’s representations and certifications have been completed online and are up to date at the time an

Offeror submits a proposal.

Question: The note for “use epoxy-coated rebar” and plan sheet 1 of 81 (PDF Page 55) does not include the pier caps and braces. Do these items require epoxy-coated rebar since they are exposed to same elements as the adjacent precast segments?

Response: The substructure elements mentioned are not required to be epoxy-coated, as shown in the plans.

Question: Can ELFHD provide existing drawings and, or the load rating, for Blue Ridge

Parkway bridge spanning over NC 18 near Laurel Springs, NC?

Response: Yes, the bridge inspection report for this bridge is being provided with Amendment 2.

Question: Will ELFHD allow the use of Cast-In-Place (CIP) concrete instead of precast elements for the piers and superstructure, or for the pier precast elements at a minimum?

Response: The superstructure must remain precast, however it may acceptable to use CIP elements for the piers, provided the contractor submits a Value Engineering (VE) proposal after the award and this proposal is approved by the Government. The contractor will be responsible for any design changes. The Pier must have the same geometry, construction schedule and environmental restrictions. Steel forms are required for CIP construction due to the improved efficiency and final aesthetics. See Subsection 103.03 in FP-14 for VE requirements.

Question: Several key personnel positions require that the proposed individuals are registered

Professional Engineers (PE) in the State of North Carolina. Is that license required at the time of bid, or can we propose personnel that are registered PEs in other States and are currently pursuing their North Carolina licensure via reciprocity at the time of bid?

Response: A North Carolina PE license is not required by the Government. However, the project is being permitted with the temporary bridges as shown in the plans. If the contractor proposes alternate designs for the temporary bridges that will result in any fill in the creek or wetlands or increased disturbance, this could require updated environmental permits. The contractor will be responsible for confirming the acceptable PE licensure requirements for permits. Delays due to licensure issues will not be approved. The RFP language will be revised per this clarification.

Question: PDF Page 18 of 211 of the RFP states the requirements for; (7) Stone Masonry

Foreman. Is it possible for the National Park Service/ EFLHD can provide a list of Masonry

Contractors that meet the requirements as stated in the RFP, or have worked on the Blue Ridge

Parkway in the past?

Response: Acceptable stone masonry contacts with experience working in this park are:

Question: Please clarify the Removal of Existing Stone Paved Waterway. PDF plan page 31 of

224 has a callout “117+07 to 119+64, remove 110 sqyd of existing stone paved waterway (LT).”

The Summaries and Schedules Sheet 3 of 8 (PDF 24 of 224) also shows this quantity in the same stationing. However, on PDF page 32 of 224 there is another callout; “118+33 to 119+40 LT, remove 107’ of existing stone paved waterway.” Aside from this overlapping with the stationing from PDF page 31 it is also a different unit of measure. Are these referring to the same Stone

Paved Waterway? If not, where is the 107’ of Stone Paved Waterway Removal to be paid?

Response: Yes, these notes and quantities are referring to the same Stone Paved Waterway. On sheet 32 of 244 (D03) of the plans, the removal of 107’ of existing stone paved waterway, 118+33 to 119+40 LT, is the minimum length the contractor needs to remove in order to construct the temporary access road. On sheet 24 of 244 of the plans, 3rd row, stations 117+10 to 119+64, we have 85 square yards of total stone paved waterway removal, which includes the

107’ long section of the existing stone paved waterway.

Question: Section 586.04 (b) Qualifications states in part "In addition, show that the pre-casting plant has performed at least one segmental superstructure concrete box girder project in the past eight years similar in scope to this project."

Ross Prestress successfully fabricated precast superstructure and pier segments for the Foothills

Parkway Project PRA FOOT 8E17 Bridge 2 in 2011. This project is similar in scope to the captioned project.

We respectfully request our performance on the Foothills project in 2011 be accepted as satisfaction for the experience requirement of this project.

Response: Yes, Ross Prestress’s experience will satisfy the precast segmental concrete box girder project experience requirement. This will be reflected under Amendment 2.

STRUCTURE EVALUATION SUMMARY

This concrete arch bridge is in good condition overall and has only minor problems at this time. These include heavy vegetation growth on the south retaining walls, exposed rebar on the underside of the arch at the southwest corner, efflorescence along fascia ring stones, and cracking of mortar joints at parapets and along south wingwalls.

Corrective measures should be taken as outlined in this report to help prevent the possible development of more serious or costly problems in the future.

• Steel two-girder bridge is in poor condition/estimated to have approximately four years of remaining service life.

(b) Construction Capability and Resources

(2) Docum. showing direct experience of Offeror, &/or subs. Submit docum. showing direct exp. of the Contr./sub in constr. precast concr. box girder segm. bridges with at least 1, but no more than 3 precast concr. box girder segm. bridge example projects compl. in past 5 yrs sim. in scope & size to our project.

(3) For each project included under (b)(2), provide owner contact information, incl.

name, title, comp., phone number, email address, initial & final constr. cost, reasons for cost changes, initial & final durations, incl. reas. for sch. changes.

(c) Technical Approach to Design & Construction:

(1) Approach to bridge construction. Describe replacement of the bridge, incl. demolition, concr. element fabrication & installation. Provide name, exp. & location of concr. segment fabricator.

Describe forms & falsework that will be used on project.

(2) Construction Staging. Describe how you will stage constr. to stay within limits shown in plans. Indicate how you will provide agricultural access to project. How you will maintain/remove temp. access. State if 1 or 2 temp.

(3) Approach to Maintaining temporary traffic control. Project requires temp. traffic control detour for over 2 yrs. Describe how you will maintain detour to min. inconvenience.

(1) Proposed Quality Assurance /Quality Control plans for constr. to deliver quality project (see Section 153). How you will approach constr. quality control.

(2) Project Schedule Control Plan - Describe means/methods by which you will plan/control scheduling of work to meet contract completion date. Discuss what measures will be taken to bring schedule back if there are nonweather related delays. Provide a conceptual schedule for completion of the project work, incorporating proposed work conditions, beginning with NTP.

(3) Approach to coordinating with EFLHD, NPS, entities such as Fire & Rescue, Police & local jurisdictions; & approach to public safety & public awareness. In order to be acceptable, Offeror’s approach to coordinating with EFLHD, etc., must include meetings with stakeholders twice per month.

(4) Small Business Subcontracting Plan (if Offeror is Large Business).

Section 107. - Legal Relations & Public Responsibility 107.10(b). J-4. Do not store fuel, oil, or other petroleum-based products on the project site. Clean up fuel/oil spills immediately & notify CO/Park Superintendent.

107.10. J-5. All vehicles and equipment including rental equipment will be inspected by the CO before entering project for mud, weeds, soil, debris, plant material, etc. Power-wash all equipment before loading for transport to the project.

Provide a 7-day notice to the CO before moving equipment into the Park.

I. Project Overview. FHWA began the meeting with a powerpoint presentation on the project given by the FHWA Project Manager. The powerpoint presentation will be included in the amendment No. 2. The project manager discussed that the presentation includes important points of the solicitation, however, the bidders should see the Request For Proposals (RFP)-solicitation document for the most accurate and up to date information. The meeting included a question and answer session.

The following is a description of the project and solicitation for informational purposes discussed directly or indirectly at the meeting.

Project Scope: The project consists of the replacement of the Laurel Fork Bridge on the

Blue Ridge Parkway, located at Milepost 248.85. The work includes removal of the existing bridge and construction of a new segmental bridge, drilled shafts, temporary bridges and access roads, mechanically stabilized earth wall, stone masonry, maintenance of traffic and temporary traffic control, and other miscellaneous work. The project is located in Ashe

County, North Carolina.

1. Question: Does the Offeror’s Qualifications Form, provided with the bid documents, get included within and count towards the 15-page limit for the Technical Proposal?

Response: The completed Offeror’s Qualifications Form can be submitted within the same envelope containing the Technical Proposal, but it does not count towards the page limit.

2. Question: The last paragraph in this Section requires (1) electronic copy on a CD-ROM. Can we provide (1) electronic copy on a USB instead of a CD-ROM?

Response: Yes, an electronic copy on a USB is acceptable.

3. Question: Can the due date for the proposal be extended?

Response: The government is working on the response to this question and the possible due date extension. Please see upcoming amendment(s) for the extended due date.
