About this file
This is a Reference Questionnaire issued by the Nevada State Purchasing Division, Department of Administration, for Solicitation Number 65DPS-S3798 regarding the Rehabilitation of Fire Investigation Burn Cells project on behalf of the Nevada Department of Public Safety/State Fire Marshal's Office. The questionnaire is due by April 15, 2026, and serves as a formal evaluation tool requiring current and former clients of proposing vendors and subcontractors to provide detailed feedback on project performance. Vendors must submit completed questionnaires to Heather Moon at hmoon@admin.nv.gov with the solicitation number in the subject line. The State requests comprehensive responses to all questions, with provisions for marking answers as "U/K" (Unknown) or "N/A" (Not Applicable) when necessary.
The questionnaire employs a 10-point rating scale to evaluate vendor performance across twelve rated criteria, including knowledge and expertise, flexibility relative to scope and timeline changes, quality of delivered materials and products, staff dynamics, schedule adherence, customer service responsiveness, billing accuracy, problem resolution capability, business requirement flexibility, and likelihood of future recommendation. Additional unrated evaluation items request qualitative information regarding the nature of the working relationship, vendor service aspects of greatest satisfaction, and willingness to recommend the vendor for future engagements. Completed questionnaires are designated as confidential business information under Nevada Revised Statutes and are not subject to public disclosure, allowing for candid client responses. The State reserves the right to contact references directly for clarification purposes.
