About this file
This is a Proposer Information and Certification Sheet (Attachment C) for an Oregon state contract opportunity related to a Smartsheet-based federal block grant financial management solution sought by the Oregon Health Authority (OHA). The document serves as the standard certification and acknowledgment form that proposers must complete and submit as part of their proposal response. Proposers are required to provide their legal business information, including company name, address, state of incorporation, entity type, and primary contact details. The RFP establishes a 180-day firm offer period following the proposal closing date, and proposers must acknowledge receipt of all addenda and agree to perform the negotiated scope of work and performance standards if awarded the contract.
The certification sheet requires proposers to affirm compliance with multiple Oregon state requirements and federal regulations, including tax law compliance, non-discrimination in employment and subcontracting practices, wage equity standards, sexual harassment prevention policies (mandatory for contracts valued at $150,000 or more), and restrictions on broadband service provision. Proposers must also certify that they are not listed on the Office of Foreign Assets Control or System for Award Management exclusion lists and that no conflicts of interest exist. The document emphasizes that all proposal contents must be truthful, accurate, and prepared independently without collusion, with false or fraudulent statements subject to liability under the Oregon False Claims Act. Additional federal funding requirements may be incorporated into the certification as applicable to the specific contract.
