About this file
This is Bid Addendum No. 4, issued by the Connecticut Department of Mental Health and Addiction Services on September 8, 2026, for project MHA-1457 involving transformer replacement at Whiting Forensic Hospital in Middletown, Connecticut. The addendum clarifies and modifies the original contract documents dated July 10, 2026, and addresses ten pre-bid request for information questions submitted by prospective bidders. The original bid due date remains September 17, 2026 at 10:00 a.m., and prospective bidders are required to acknowledge receipt of all four addenda on the Bid Proposal Form Section 00 41 00 or face disqualification. No site visits or pre-bid conferences are referenced in the addendum.
Key clarifications provided in the addendum establish contractor responsibilities for survey costs (including pre- and post-construction surveys with self-established control points), soil characterization and disposal of excavated materials with reuse prioritized on-site, and groundwater management if unexpectedly encountered during excavation. The State retains responsibility for testing and abating groundwater remediation wastewater discovered during construction, with a four-day response requirement for testing requests and ten-day disposal arrangement requirement. The addendum confirms that temporary compacted aggregate base material may be installed with a 30-day cure period before permanent pavement restoration, specifies that pre-cast bollards are not acceptable and must conform to detailed specifications on Sheet C-2, and clarifies that transformer pad sizing must follow the manufacturer's recommendations as shown on the drawings. Line striping restoration is excluded from the scope of work, and existing site line striping is identified as minimal.
