About this file
This is a Minority/Woman/Small Business Enterprise (M/W/SBE) Utilization Form required by the City for submission with proposals related to water treatment valve rehabilitation services in North Carolina, specifically for Charlotte Water operations throughout Mecklenburg and surrounding counties. The form serves as a mechanism for companies to document their commitment to including City-certified Small Business Enterprises (SBEs), Minority-owned Business Enterprises (MBEs), and Woman-owned Business Enterprises (WBEs) in their subcontracting efforts. Bidders must identify any M/W/SBE certifications held by their own company, provide details on outreach efforts undertaken to maximize M/W/SBE participation during the proposal phase, and describe strategies for continued M/W/SBE inclusion throughout the contract performance period.
Proposing companies are required to list all intended M/W/SBE subcontractors, providing the subcontractor name, description of work or materials to be provided, relevant designation letters (M, W, and/or S), City vendor identification numbers, and calculated utilization percentages for each M/W/SBE category and combined M/W/SBE utilization overall. The City has established a strong policy commitment to M/W/SBE inclusion when viable subcontracting opportunities exist, and companies are expected to demonstrate exhaustive efforts in identifying and engaging qualified M/W/SBE firms. The form requires authorized representative signatures and submission as part of the complete proposal package.
