About this file
This is a Non-Conflict of Interest certification document for the Colorado Department of Corrections related to the YOS-Residential Community Transition Services contract opportunity. The document requires authorized signers to certify compliance with Colorado Revised Statutes sections 18-8-302, 18-8-306, 18-8-301, and 18-8-308, as well as the State of Colorado State Controller Policy on Procurement Conflicts of Interest. Offerors must affirm they have no actual, potential, perceived, or apparent conflicts of interest and acknowledge understanding that the Procurement Official has authority to remove them from the process. Signers confirm they may have access to State information and records, which must not be distributed, sold, shared with third parties, or used except as expressly authorized.
The certification further requires offerors to declare they have no personal or beneficial interest in the services or property described in the contract opportunity and commit to immediately notifying the Procurement Agent if any situation arises that could alter their representations or create the appearance of conflict or impropriety. The document specifies that signatories are either employees of a private employer or independent contractors under contract with the State of Colorado and acknowledge receiving information in that capacity. The document requires signature and printed name of the authorized signer as well as the company name or doing-business-as designation.
