About this file

This is a Contract Performance Disclosure form from the Colorado Department of Corrections required under Colorado Revised Statutes 24-102-206. The form serves as notice to the Department of Personnel when a governmental body awards a contract to a vendor that will perform services outside the United States or the State of Colorado. The form must be completed and signed by the contractor's President, Board Chairperson, or authorized signatory and returned to the sole contact listed on the solicitation. The disclosure requires vendors to indicate whether any services under the contract or subcontracts will be performed outside the United States or Colorado, and if so, to provide detailed information regarding the type of services, specific locations where services will be performed, and justification for performing work outside these jurisdictions.

The form requires vendors to provide comprehensive details about any out-of-state or out-of-country service performance, including the rationale for why such arrangements are necessary or advantageous. Procurement personnel must verify that vendors provided required notice within 20 days of contracting or deciding to contract outside the United States or Colorado. This disclosure mechanism applies to non-construction contracts and represents the state's transparency and oversight requirements for vendor performance locations. The form does not address pricing, set-asides, or funding sources but functions exclusively as a notification and disclosure tool to ensure state visibility into contractor operations.
