About this file
This is a Bidder's Disclosures and Certifications form for the City of Alpharetta, Georgia, relating to Procurement Number 26-117 for On-Call Storm Pipe Cleaning services. The form requires bidders to disclose any conflicts of interest, financial relationships, or campaign contributions involving City elected officials or employees. Bidders must certify that neither they nor their principals are debarred, suspended, or proposed for debarment by any federal, state, or local entity. The certification also requires verification that the bidder has not been convicted of fraud, criminal offenses, antitrust violations, embezzlement, theft, forgery, bribery, or related crimes within the three years preceding the bid, nor have any contracts been terminated for default by any public agency during the same period. The bidder must affirm that the proposal is submitted without collusion, fraud, or prior agreement with other bidders and that employees will not engage in business ventures or provide gifts or gratuities to City employees or consulting engineers.
The form mandates compliance with state and federal Equal Opportunity and Non-Discrimination requirements and obligates the bidder to certify that provisions of the Official Code of Georgia Annotated, Sections 45-10-20 et seq., have not and will not be violated. This Georgia statute prohibits City elected officials and employees from transacting business with the City in which they are employed for personal benefit or on behalf of businesses in which they or their family members have substantial interests. The form must be notarized and signed by an authorized officer or agent of the bidding company, with all required information completed and submitted as part of the procurement response.
